New PI combination (one pill once a day treatment)

Gilead and J and J announced today they are teaming up to introduce a new combination pill containing Prezista and Cobicstat (new booster). 

They are also negotiating terms for developing a new single tablet regimin co taining Prezista, Cobistat, and Emtriva and GS-7340 (a new tenofovir formulation). 

Good news all around for the simplification of treatments!!

That said, I could potentially switch to this as I'm already on that combo, and then one more pill (Isentress) as I'm on a strengthened regimen containing one extra med due to old resistance issues.

btw, this new GS-7340 thingie evidently is much more potent than tenofovir, and consequently much more safe in terms of toxicities. Somehow I'd not even heard about this development until now

I guess if this refers to bone density issues then it's a great thing seeing as how so many people use tenofovir in some form.

It appears to say it is likely safer on the kidneys but might potentially be a bigger problem for bone density -- but they plan to study it.

If so, GS-7340 could theoretically harm bones. William Lee, PhD, from Gilead replied that the company has not yet looked into this, but that they would do so in the future

No, I'm not on it. I had a discussion with my doctor sometime last year because some of the Phase 3 trial was being done at my HIV clinic, and it's his opinion (or was at that time) that there's little difference in the chemical make up of this and Norvir. My questions at the time were more aimed at GI issues, not cholesterol -- why has your doctor indicated that it's better for that? (thanks for the bone density thing -- obviously I was skimming :)

People should keep in mind that while a lot of these new Gilead developments are nice in terms of dosing, and some marginal improvements in performance, that much that is driving things is that a lot of these meds come off patent in the next five years or so. Or in the case of the new booster it's easier than doing new once a days and going through Abbott.

Just to be clear, this isn't that "quad pill" I've heard some people talking about, is it?

No, its a PI + the new Gilead booster, the quad pill contains the novel integrase inhibitor elvitegravir plus 2 other active meds and the new booster.

Plus there will also be the new Edurant-based once-a-day tablet (also a Gilead/J&J joint product) so in total there will be three new all-in-one pills by sometime in the next 12 months or so.

whats the skinny on these pi's and the nasty sides. thought i read on here many times that pi's are more prone to the nasty sides?

I've not found Prezista to have side effects. I've been on it for five years.

I'm soo excited for us to have more choices for possible pill burden reduction and expanded treatment options.

Am also proud the level of collaboration and cooperation posters are sharing for information.

I really admire Miss P, Ann, Leatherman, Newt, Gymsorat ..... too many name but well deserving of a mention.

May we all remain healthy, wise and receive the best possible care we can ... as we advocate for ourselves and others with our disease processes.
