Lipoatrophy after 6 months on Truvada / Reyataz / Norvir

Hi,

I'm noticing it mainly in my face and my arms.  It's very obvious on my arms - I've lost at least an inch around my wrist and my arms just look really thin. 

I've had other symptoms, muscle spasms, muscle weakness, virtually from Day 1.  (I was on Isentress / Truvada for the first six weeks, but switched to above - didn't change the muscle spasms or weakness problem though.)

Has anyone else experienced this and had success moving to another treatment?  Success meaning the lipo doesn't get any worse than it is now.

Seems like a quite fast and dramatic development. Has this lipoatrophy been diagnosed and measured by your doctor?

Hey Ryoma,

I agree, that would be a really fast onset for lipoatrophy.

This would be highly unusual, especially since you started with the regimens you have been on.

Isentress isn't known for causing lipoatrophy, or lipohypertrophy for that matter. Neither is Truvada.

While the PIs have been known to cause lipodystrophy, Reyataz is the least likely to cause this. The minimal amount of Norvir you are taking to boost the Reyataz shouldn't be a major player either.

All of the above is "should," not can't. We are talking about HIV meds here.

All of them have side effects, or could have side effects, or have had side effects, or we don't know if they will have side effects in the long run.

The first thing you should do is call your doctor and set up a face-to-face with him or her. 

The doctor needs to be aware of the symptoms you are describing and your concerns and observations regarding lipodystrophy.

Regarding other regimens, you have already been on the most lipo sparing of them.

Let us know what your doctor says.

Thanks everyone,  I saw the doctor today, he says we need to wait and see for a little bit more - for the exact reasons noted above.  And to check if it couldn't be anything else.

If it really is lipoatrophy, though, any idea what my options would be?  The doc said it most likely would be the Truvada, if that's the case he said swapping out to another NRTI probably wouldn't help.  Does one dump the entire NRTI class for this type of problem?

(I was on Isentress / Truvada for the first six weeks, but switched to above - didn't change the muscle spasms or weakness problem though.)

When I tried Atripla, I got muscle twitching and leg cramps because the CNS effects caused me to void like crazy and I let myself get dehydrated. I lost 6 lbs in one week because of it.
When my HIV/IDS switched me to Isentress & Truvada the excessive voiding stopped and so did the twitching. I drink at least 64 oz of water daily and sometimes more when I am running. I also eat two bananas a day for the potassium.
Could there be a chance that you are dehydrated?

Probably not, I eat and drink regularly, no issues with diarrhea at all.  In the past month I've started feeling nauseous periodically, but not enough to prevent me from eating properly.  I can't help feeling that the muscle spasms, weakness and the (perceived) atrophy are all related but none of the tests so far or the doctor's experience points to anything in particular.
