6 mo. VL 241 on Dubious Combo

After becoming resistant to efavirenz (sustiva) and one of the components of Truvada (emtricitabine or 3TC), I was shifted to Reyetaz (atazanavir) boosted with Norvir, Ziagen (abacavir), and tenofovir (TDF).  I later learned this combo is dubious.

At one month in my VL was around 1,200 (if I remember rightly) down from 80,000.  Then at four months in it was at about 240.  Now the most recent test at about 6 months in is 241.  

I have only spoken to the nurse who gave me the results as the doctor was unavailable, but she told me that the number was 'too high'.  At the recent visit the doctor told me anything other than undetectable would be bad.  I will be going in for a follow up in about 2 or 3 weeks.  

I'm very concerned as I think resistance is extremely likely from what I've read of this combo (abacavir and tenofovir).

Took me 8 months to get undetectable on this combo. If it stays around 250 I wouldn't worry.  But I agree, there's some way to go to the magic 50, and this is desirable. you really need a decent genotype in these situations.

If your viral load rises over 1,500 in the next test I would worry.

Are you sure about the components of Truvada ?

Yes, I'm resistant to the one which is not tenofovir, so I guess that's emtricitabine or 'Emtriva'.  It is always confusing with so many names for these things.

Yes. I apparently have the M184V mutation that makes me resistant to Emtriva, but extra susecptible to tenofovir.

Wow, thanks!  Its a relief to me to speak to someone who is on this one - it seems to be rare.  I hear so many bad things about the abacavir tenofovir combination.

Anyway I'm scheduled to go in in a week or so and see what if anything they have to tell me..
