Article HIV hides out in peoples intestines.
« on: July 29, 2006, 07:40:51 AM »
This seems interesting a new discovery that hiv hides in intestines! Maybe this could lead to new treatments.

The HIV virus hides out inside people's intestines, researchers said on Saturday in a report that offers new understanding of the incurable infection.

The virus replicates in the lining of the gut and does much of its damage to the immune system there, Satya Dandekar, chairwoman of the Department of Medical Microbiology and Immunology at the University of California Davis Health System, and colleagues reported.

Writing in the Journal of Virology, Dandekar said the study was the first to explain why the drug cocktails taken by HIV patients so often fail to work completely.

"The real battle between the virus and exposed individuals is happening in the gut immediately after viral infection," she said in a statement.

"We need to be focusing our efforts on improving treatment of gut mucosa, where massive destruction of immune cells is occurring. Gut-associated lymphoid tissue accounts for 70 percent of the body's immune system. Restoring its function is crucial to ridding the body of the virus."

HIV cannot be cured but the drugs, known as highly active antiretroviral therapy, or HAART, can keep the virus under control.

At first, doctors had hoped that years of treatment might eventually eradicate the virus, but, 25 years into the epidemic of AIDS, it is clear that cannot happen. That is because the virus can hide out quietly in reservoirs, which include certain immune cells.

The gut is clearly important, too, Dandekar's team said.

"We found a substantial delay in the time that it takes to restore the gut mucosal immune system in those with chronic infections," Dandekar said. "In these patients the gut is acting as a viral reservoir that keeps us from ridding patients of the virus."

The mucosa are the wet tissues that line the nose and throat, the genitals and the inside of the gut. HIV often infects people via the mucosa.

Dandekar's team has been studying HIV-infected patients who, even without treatment, have survived more than 10 years with healthy immune systems, including the T-cells that are attacked by the virus.

"We looked at their gut lymphoid tissue and did not see loss of T-cells there. This correlated with better clinical outcomes," Dandekar said.

So they started the current study, following 10 patients being treated with HAART, taking blood and gut samples before and after three years of treatment.

They found evidence of inflammation, which disrupts tissue function, promotes cell death and upsets the normal balance of gut bacteria.

Dandekar said these findings suggest anti-inflammatory drugs may help HAART work better.

Then I was correct in saying, "No better than some of these meds work, we should stick them up our ass."

There seem to be a lot of gut issues lately. Being gluten intolerant, I am fully aware of the lymphatic system that exists in the intestines.

Before diagnosis, I unknowlingly spent years with intestines that looked like a war zone. Because it was an autoimmune response, my body was literally attacking the villi of my intestines when gluten was detected.

The lining of my intestines looked like a congealed mass. It took more than a year being gluten free for them to begin to recover.

Now, knowing that HIV is lurkiing there, I wonder the combination of the two didn't do me in. I do know there is more HIV found in anal secretions than in the blood. Anal secretions are the mucous found in the large intestine produced to help move along fecal material.

The more we learn, the more we realize there is much more to learn.

HUGS,

Mark

I know this is a very serious subject and I pray the research leads to better treatments....but why do I keep thinking of Richard Gere?

Hey Rion  I wish I was over there, all I can find in NYC is powdered whey- what the Dutch call melk water, I am sure that is good for the system too. I think it is not just what you eat but the quality. Which is why I  make sure there are no extra sugars in the yogurt.

I thought HIV could infect and hide out almost anywhere in any cells; perhaps just more so in a certain enviornment; maybe the filthy little bastard likes filth swimming in it's own sh*t, or ours. ;D

This makes one wonder if the diarrhea so many of us suffer from is due to more than the meds.

My thought actually went to whether or not GI related side affects can affect CD4 counts.  If your guts are being ravaged by meds, that mucosal lining has to be affected and/or damaged to some extent.

Congrats on your recovery Mark.  8)  This might sound bizarre but I think it's almost  a privilege to see what you saw.  8)  8)

I'd like to see that in my own system.

If anyone has image links (science-ish links) to a normal or HIV+ mucosal lining please send to me. Thx

Shane, that's a great train of thought.... I wonder if any laboratory studies have been done to find out the correlation (if any) between "slow to rise" CD4 persons and their frequency of diarrea/nausea!

I think people are using the term 'hide' to mean two things here. The virus can replicate in areas that don't get sufficient drug coverage (drug privilege site) or it could integrate into a quiet area of a cell's genome and not replicate. The latter sort of 'hiding' is latency and can probably happen to cells everywhere. The former will only occur in a specific place.

You should never make clinical decisions based on what anyone says on the internet but rather check with your ID doctor first. Discussions from the internet are just that - Discussions. very interesting indeed, I have always had trouble with my digestive tract, this could explain why I have a low CD4 after a very short exposure, intestine removal could save your life !!!!!!!!!!!!!!!! people with crons disease often have to have it removed...they survive

A few years ago I had chronic diarrhea and lost 35 lbs. To be graphic I would take my Kaletra and 15 minutes later the capsules would arrive intact in the toilet bowl. The doctors did all kinds of tests, endoscopy, colonoscopy, etc. and came to the conclusion that the HIV virus itself had "shut down" my digestive system. Fortunately it got better on it's own.
