Hiv and nurosyphillis

Hey all, have just got back from the clinic again - god it seems like i am there every week at the moment,

My doc thinks that due to my hiv infection it has re-activated my syphillis she also mentioned that this is quite serious and could progress quicker to nurosyphillis am not sure what the numbers mean but the big number has jumped from 1260 to 2160 and the small number has dropped from 16 to 8  she was not totally sure if my syphillis levels had increased so asked me if i wanted to be treated for syphillis now or have  further bloods done

as i rememberd how painfull the injections are i opted for further bloods!

Also had a resistance test done, vl, cd4, and something other test to see if i am ok to take abcivir not sure what that was called

in total i had 12 vials taken!

Hey m8,

Have you received the appropriate treatment for Syphillis? I had 2 injections of penicillin in each butt cheek for 17 days straight and that did the trick. I didn't know HIV infection could re-activate Syphillis, I just thought it can cause a false positive Syphillis result.

What a fucking nightmare eh?

Apparently, HIV impairs immune system function that keeps syphilis contained.  When my partner's numbers took a dive four years ago and he had to go on meds, his syphilis tire was way up.

Do you know which test she did for the syphilis? Usually the classic test is called RPR, and it returns with a ratio like 1:2, or 1:256, or 1:2048.

The ratio is the amount of dilution necessary before the syphilis gets undetected in the vial. So a ratio of 1:2 is excellent, a ratio of 1:2048 is bad, because it the vial needs to be diluted 2048 times before syphilis is not found.

Usually a ratio of 1:512 is the point where you may undergo a more aggressive treatment, that is high doses of penicillin in IV or more than 4 intramuscular (the famous butt shot, which hurts as hell, I concur) in a two week period. Then new RPR tests are done and the ratio must show an improvement, that is the number on the right going down.

Syphilis is no joke if going untreated, so make sure this is on your priority list.

milker  i have had the rpr test and it has increased since my hiv infection - notes from the mircobioligy dept was the recomended three courses of the injections  - i had two shots when i first got syphillis so i am trying to avoid them this time that is why i asked for the test to be reconducted.

i know i will have to have the injections but i couldnt face them today so asked for a re-test

obviously i dont want it turning into nurosyphillis

Thats where I was getting confused. People with just Syphillis only have to have 2 Injections I think? But when you are cross infected with HIV, you need the full 17 days worth (Or there abouts). To be honest, I didn't find the injections that painful, and was fucking glad when I had finished them and it was confirmed I just had dead Syphillis anti-bodies, which I'm pretty sure you can never get rid of.
It is a nightmare having that treatment, but doesn't even compare to the thought of NeuroSyphillis in 15 years time.

The general rule of thumb for measuring treatment progess is that three months after the conclusion of treatment, the RPR titre should have reduced at least 4 fold, eg a titre of 1/16 should have become 1/4 or lower.

Many people will return a titre of 1/1 after successful treatment, HIV or not. This is OK.

matty am gona phone up my clinic tomoz to get my defo rpr variations since sero conversion ill post them on here and we can discuss, nearley fainted last time i had the injections and had to be held down so only want them again if its absolutley neccasarry

Harry, the number of injections may vary, depending on the starting RPR result. Some people also undergo a spinal tap to see how much syphilis has gone through the cerebro-spinal fluid.

Matty is correct, and if your RPR results has increased, then it's time to take this seriously, get the butt shots or you're going to have to go for IV. I know they hurt, been there, done that... You can ask to be put on a bed so you can bite the pillow.. or some hot nurse, maybe :)

I has syphillis 5 years ago and then had rapid progression to some neurosyphillis - a spinal puncture was the diagnostic test which showd the infection had passed the blood/brain barrer.  I did 21 days of two shots a day. Not a nice period.  In the Uk the default now is just two shots even for co-infected.  Treating early has to be the best option, and ask/discuss if the low shot option should be tried first. 

I went through hell with daily heahaches and undiagnosed syph for 3 months. so at least you have a diagnosis and treatment options now.  My headcahes stopped after the second day of shots.  I say dont wait until there's any more infection.

I had it too...I went through three series of the shots, then 14 days of IV - 24 hours a day. God that was awful. I had to wear a CADD pump around my waist in a fanny pack everywhere I went including to work. Talk about awkward!

Anyway, NEWLAD, I am sure you will be fine...and remember the lower the ratio the better you are!
