General Skin Irritation

I've found a lot of postings on penile skin irritation, rashes as a result of medication, etc. and I could have sworn I found one on HIVers in general having skin problems.

And I'll be damned if I can find it now.

I was diagnosed approximately 6-7 weeks ago now, and I have yet to see my ID doc.  They canceled the first appointment!  My skin continues to get more irritated in several places and it's to the point I'm considering a dermatologist.  I have two questions for everyone:

1) Are skin problems common amongst treatment-naieve HIVers? Both general skin problems and dandruff.
2) What, if anything, do you use to help the problem?  I use selenium and skin cream, neither work.
3) Is it something worth taking to a dermatologist?

yes, i have skin problem and dandruff problem too. I was diagnosed in august (seroconverted in Feb) and not yet on any meds. My dandruff problem is now better after using selenium and it takes a while (3-4months to completely recover), so be patient. Also, my skin is drier and got whiteheads on my face. I know my body is adjusting and now gradually (after 3 month since i diagnosed) it is getting better also as i changed my face cream. I think it is quite common, i know other people have similar problems.

i hope this helps..and dont get stressed too much, it could only make things worse.

Yes, very common. I am currently seeing a dermatologist who runs a special clinic for HIV-related skin problems. I have been battling folliculitis ever since seroconversion but there's finally signs the treatment is paying off.

In your case I think seeing a dermatologist would be a good idea - perhaps you ID doctor can recommend someone.

Until a diagnosis can be made the best advice I can give is to avoid soap, shampoo, etc containing SLS (sodium laureth sulfate). It is a very common ingredient but it is also a known irritant, especially to sensitive skin. You can often find products without SLS in health or organic shops. Someone on here gave me that advice soon after I joined the forums and it really helped (although it didn't entirely resolve my problems).

Mesu is right, just from experiences, I do find that, pre- or post-diagnosis, the more dandruff and general skin irritation I get when I am more stressed. When I had to stay late at work for a whole week for example, no matter how much I washed I hair, the dandruff could get so bad that I called it a minor snowstorm. At this early stage it's difficult not to get stressed, so just try to use more mild hair conditioner and for guys, even if you're not used to, go get some face cream.

Just for two cents.. and of course if it gets really serious with other skin conditions talk to your doctor..

i have noticed minor skin irritations here n there..nothing major, but being that i have never struggled with bad skin it was very noticeable to me..i have really clear (pretty skin some say) but sometimes i get lil circular dry spots on various parts of my body..mostly legs..only about one or 2 but its still weird looking and itchy and im a picker so ill scratched it n pick it until it hurts..(i know ima werido, love to bust other peoples bumps too)..ok TMI..but anyway yeah its def related to the virus itself..i was dg over 4 years ago now...Eucerin, and lubriderm are pretty good with them...

Matty the Damned is plagued with skin problems.

Usually it's just dermatitis but over the last couple of months I've had these sores appear on the sun-exposed areas of my skin (upper arms, legs, face) which are intensely itchy and take literally weeks to heal.

No they're not cancers, I had that checked.

Mini had such bad skin, especially on her head, that her hair wouldn't grow.  It was just one great giant flake.  Her skin gets really "ashy" and itchy.  We keep cocoa butter on hand.  Her hair has also taken a turn.  PID says that meds come out through the skin and the hair and that's where her problems are coming from.  Someone has suggested deleting soaps, etc.  Our dd #3 has depeleted barrier cells in her skin.  We've stopped using soap (use Eucerin instead), lotion, wet wipes (she's not potty trained), and only Tide and Downey free for washing clothes.  When she goes outside to play, she has to be covered in long sleeves, hat, pants, socks,and shoes.  Makes for crazy looks at the beach.  We've also stopped putting her in fleece because it dries the skin.  It's a lot of work, but it does make things much easier.  The rashes stay under control and so does the horrible itch.

Well I have had problems with my back and shoulders and upper arms. I think maybe its the water but i do not know. If I take my fingers and rub them with a little force across my shoulders or back or upper arms I can rub skin right off. It's as if it was dirt that you can rub off, but it is skin though it does not hurt it just rolls off in little balls. Every day. I have changed soaps too. I also get eyebrow dandruff right between my eyebrows. I can scrape it off with a sharp object.

Once I got a massage with oil on my back and even that caused shin to roll off. I use a scrubber on a stick with soap and it seems to get it all but the next day it is there again. I dont know what to do about it.

Boy howdy, skin problems are way common with the HIV set.  I have recurring problems with dermatitis and dandruff.  I use ketoconozole shampoo to help take care of it.  Requires diligence, as it will always return soon if I stop using the stuff.  Have the itchies right now from some dermatitis, actually.

I have a lot of "itchy spots" here and there.  They're never in the same place.  One day it's my back.  The next day it's my legs.  The next day it's my arms.  I use a LOT of lotion now, which is something I never did in the past.  It's just another tragedy of HIV, I reckon.  A damn cat hopped up in my lap about three weeks ago and scratched one of my thighs.  The mark is still there three weeks later.  That ain't cool, and it's one of the many reasons why I don't like cats anymore.

Unfortunately skin problems are common with HIV as I've found out.  I keep my dry scalp more or less in control with Selsun Blue daily and Nizoral twice a month.  On my face the dry patches are controlled with Derma-E cleanser daily, with Elidel if they get bad (prescription) and on my body beta-methasone.  My derm said "moisturize, moisturize, moisturize" so I do with Eucerin face cream and Aveeno for the body.  Skin care is a bitch.

I've heard the skin problem "might" go away when I start meds, we'll see.

Go to the dermatologist. My ID Doc told me to go and they work together on treatments. I have been doing this for about two or three years now. The eyebrow dandruff is the worst! It is irritating as hell. I have to clean my glasses a couple times a day. No matter what I use it just won't go away. I also get the dermatitis problem. I can't stand the breaking out then weeks to clear up. There is a new lotion which came out a couple of months ago called "Cer-Ve" and it works great. It also costs about $14.00 per bottle.

First off, I want to thank everyone for their response(s).

Based on the feedback I am getting here, when I visit my ID doc for the first time next week, one of the first things I'm going to ask about is a dermatologist, preferably one that is experienced with HIV patients.  It seems that the shampoo recommended is available through a dermatologist, so that gives me something to ask him/her once I see her.

I'll stick with the selenium, too.  I had been using the Jelly Bean thing to practice taking pills, but I stopped once they canceled the first ID appointment (after 2.5 months, I got the gist I could do adherance of Jelly Beans at 90%+ and that HIV medication might be easier because of the actual stakes.)  I may start this up again using the selenium until such a time that I'm prescribed meds.  Up until now my selenium usage has been somewhat erratic. 

Sodium Laureth Sulfate... found both my dandruff shampoo and my hand cleansers had it.  I'll switch to Johnson's baby shampoo, a shorter haircut, and using the CeraVe (which appears to be available through the major drug store chains) ASAP.  I'm applying for new jobs over the next 3-4 weeks and I need to get rid of the flaking...and wear lighter colored clothes.

If anyone has suggestions on more products to use/avoid based on their personal experience, please post them.

I've been having a HORRIBLE case of dandruff/dermatitis lately. Doesnt matter with what i wash my hair i still cant get rid of it. I have tried everything, guess ill have to ask my doctor about it next time but i always forget hehe.

Have to chime in here...  me too.. skin problems... have been seeing a Derm Dr for several years.....the Cera.Ve works and so do several other similar products....but the thing that got most dramatic effect was to not use shampoo very often...during daily shower wet hair "mess it up "  then rinse again   no shampoo.....maybe once a week shampoo...haven't had the dandruff issue for a year....other areas  face, arms, knees...Cera .Ve  and Cetaphil work great...

May want to change what you're washing your clothes with, too.  We switched to All Free and Downey Free.  We also use baby oil to seal in moisture.  Derm said in winter to use a humidifier to keep the air moist since the heat dries it out.  Also, stopped putting dd3 in fleece since it dries out the skin, also.  She's also on the no soap (we like cetaphil), wet wipes, or lotions.  And nothing containing alcohol - like cootie killing sanatizer stuff.

Nothing scientific to post actually, but I did notice that when my VL was higher my skin was dryer and I had dandruff issues.  Now that I am healthier, my skin is still dry but nearly as bad and my dandruff went away.  Your skin is a giant organ so maybe it is related, who knows.  As for me, I use a lot of vaseline skin care on most parts of my body, a good facial cream (not that drug store crap) like clinique or lab series .... that and tons and tons of water seems to keep my skin decent now.  However, I can tell when I am not drinking enough water because my skin gets worse.....plus water helps flush all that med shit out of your body .... at least that is what I tell myself as I spend half the day pissing.

Hi.... I've had the dandruff problem too, as well as the "flaky" skin at my eye brows, mustache, goatee, etc.   My Doc prescribed Ketoconazole cream for my face and recommends anti-dandruff shampoo daily.  It seems to help alot, tho it is itchy until the flaking calms down  ::)

I've just recently gotten a weird skin condition myself, My dr looked at it and rulled out scabies, as well as the big Syph. as well as chicken pox. He thought i might have picked something up from a hot tub but i havent been in one since this summer.  It's  weird kinda patch of red bumps on the back of my legs and some on my chest and a few here and there on my arms.  He said that skin issues come and go with HIV and it'll pass. He put me on a ten day double antibiotic. it seems to be clearing up but not all the way.. I'm keeping my fingers crossed...these fuckers itch at night :)

Skin issued started about a 4 months after conversion. Like little bumps on my chest, back and shoulders. I take a pumice stone and rub them off in the shower daily. Also with the dandruff/dry scalp I found type of hemp or aloe based shampoos and conditioner actually gets rid of it. I used to by it in Key West and Austin, but now that I dont travel as much I found it online. Works amazingly well for me, maybe it can work for yall too. No itching or flaking anymore :) Not even the Selenium stopped it for me before. The Company that makes the stuff is called Nature's Gate. They have a website where it will direct you where to buy it.

I haven't seen a dermatologist yet, but I have seen an ID doc.  I'm relieved the couple of patches of really bad skin I had were not KS.

He suggested and prescribed a few things.

1) He suggested either the Ketoconazale shampoo or Selenium Sulfide shampoo (Selsun Blue) once a day and not once or twice a week.
2) He felt my skin problems were probably a fungal infection and gave me a prescription for Clortimazole and Betamehason -- a combination of an anti-fungal and a steroid.
3) A course of 1mg Valtrex tablets twice a day for that second "gift" I seemed to receive.  It was the receipt of this that led to my HIV test.

It looks like I'll get my first numbers tomorrow or likely Wednesday given the holiday.

I have not been using the shampoo each day until this point, but it seems to have some positive effect, so thanks for all the suggestions, as well as that one in specific.

I'm posting this follow up in case it's of use to someone later.

Within 3 days of using the cream (Clortimazole/Batamehason) most every red, itchy patch of skin that I had started to disappear, and now a week later you'd have to know where to look to even determine that I might have had a skin problem in a certain area.  I keep very close-cropped hair and used a bit of cream at the scalp line and one or two trouble spots and BOOM, everything is much better.

My dandruff overall is probably 10-20% of what it was last week.  I've used the shampoo a few times but not every day yet  It's gone from a blizzard to a light dusting, though I'd still avoid dark colored shirts. ;) 

I've now started to use the cream for some athlete's foot that I have too as that has gotten much worse over the last month.  Just after a day it feels much improved in terms of itching and cracking.

I assume most of this is the cream, but I'm sure the Valtrex hasn't hurt matters either.

No numbers yet (dammit!)


I haven't seen a dermatologist yet, but I have seen an ID doc.  I'm relieved the couple of patches of really bad skin I had were not KS.

He suggested and prescribed a few things.

1) He suggested either the Ketoconazale shampoo or Selenium Sulfide shampoo (Selsun Blue) once a day and not once or twice a week.
2) He felt my skin problems were probably a fungal infection and gave me a prescription for Clortimazole and Betamehason -- a combination of an anti-fungal and a steroid.
3) A course of 1mg Valtrex tablets twice a day for that second "gift" I seemed to receive.  It was the receipt of this that led to my HIV test.

It looks like I'll get my first numbers tomorrow or likely Wednesday given the holiday.

I have not been using the shampoo each day until this point, but it seems to have some positive effect, so thanks for all the suggestions, as well as that one in specific.

OK, I have read through this again, and I am a litte confused. I am starting...well been having pimples in the middle of my chest. Now it seems to be getting worse and the gel stuff I use on my face isn't keeping up with it. I plan on going to the ID doc next week. Any suggestions before them or some home remedies before I head in the to the doc to ask about pimples?
