Why is this a surprise or a discovery?  (I thought it has been known for years.... )

I think this is the "money shot" from the report that is newsworthy to the microbiologist researchers:

Digging deeper, the researchers found the inflammatory response through IL-1β production was initiated in Paneth cells, which are known to protect the intestinal stem cells to replenish the epithelial lining. This is the first report of Paneth cell sensing of SIV infection and IL-1β production that links to gut epithelial damage during early viral invasion. In turn, the epithelial breakdown underscores that there's more to the immune response than immune cells.

anyone who has been poz for more than a few yrs knows this. ;) I guess the science is finally just catching up to what is already known by pozzies, ID docs, and ASO clinics.

This explains why Boston always wants to schedule an upper and lower endo, and sighs heavily whenever I say, "Sorry, just a leukapheresis this time, please."

The latest find that is related, is that benign bacteria particles are "leaking" from the gut and is likely a significant cause of inflammation associated with HIV even when the virus is controlled.  Renleva seemed to help.

How does one measure the damage to an HIV+ person's gut? There must be a sliding scale, from none, to little, to some, to a lot, to debilitating and so on....

Mecch, two words:
rectal biopsy
:P

Researchers can also measure bacterial products (e.g., lipopolysaccharide or LPS for short) in the blood.

We've really known this as far back as 2004-2005 in some form or another. The virus loves hanging out in mucosal tissue (gut, sinuses, etc.), where most CD4's live.
