URGENT ADVICE NEEDDED ABOUT THE PEP

hi,
 my partner had a condom burst case about 3 hours back and without wasting time we want to get PEP started . please adivse  the right dose of PEP. As I have heard it is duovir for 30 days. I will be greatful for immediate advice.

You have to go to Emergency at your local hospital or call your doctor to discuss this. Course of treatment is usually 28 days.

FYI, for testing and getting a final, reliable result it's 13 weeks from completing the PEP treatment, although you can get an interim result at 6 weeks after.

hi,

 i understand your advice that we should contact the doctor immediately, but unfortunately we are travelling from main city to a far off place where the doctor on hiv is ot available and it will take atleast 3to 4 days to reach to a competent doctor. therefore i request once again to advice  abut the name of medicine , its dosages. i have medicines avaialble with me for a weeks time till i get to main place. iam on effervinez and duovir. so please please advice us abut the course of action to be followed as we are really worried. thanks for the help.

Alerthari,

As a person living with HIV (you I mean) you may consider also posting this in the living with forum and the treatment forums to get more responses.  Very few of us check this forum.  I cant offer you any advice on dosages and what not but there are some who may be able to give you more guidance in that area on the other forums.

I am curious tho, I am assuming it broke while having intercourse with you.  And that you are positive and he is negative.  And from the sounds of it that he was the insertive partner.  Did you guys stop immediately when you felt it break?  Or did you not notice until you were finished (post ejaculation)?

Duovir is generic Combivir which contains Lamivudine (3TC) 150mg and Zidovudine (AZT) 300 mg.
Dosing is 1 tablet every 12 hours.
PEP needs to be started within 72 hours of exposure and taken for 28 days.

For serious exposures (needle stick), the U.S. Public Health Service recommends using a combination of three approved antiviral drugs for four weeks. For less serious exposure (in your case a broken condom), the guidelines recommend four weeks of treatment with two drugs: AZT and 3TC.

hi,
 yes you are right that it broke while having intercourse. i was insertive and am positive n my partner negative. we did nt realise till i removed . then i saw that it had got burst. my techological skills  not vey high ; so i dont know how to shilft this topic in living with hiv. may be one of the moderator can help me out in this regard. once again i appeal for help on adivce about PEP ,  name of medicine , dosages. will combivir be of help and if so in what dosafges and its frequency

 I would add also that since it appears that you are likely on medication at this time yourself, there is already a lowered likelihood of infection. I've used PEP myself before however and trellium is quite correct about the guidelines for less serious exposure such as yours. Try to keep calm. I would suggest that you still speak with someone directly within the medical establishment. You can always go to the local "free" clinic. Often they'll avail "starter" meds for say two or three days or maybe even until you are able to contact your personal doctor directly. If nothing it might help to allay some of yours and/or your partners fears.

Hey Alerthari,

I received your PM about this matter but I'll reply here in your thread.

I agree with Andy. Your partner was protected until the condom broke and the best idea is for your partner to seek assistance regarding PEP at the emergency department of a hospital. Remember too that the window period for testing is 13 weeks from when he stops PEP, if indeed he decides to take it.

By the way, you don't have to call me 'Sir', my name's Matty. ;)
