Title: Positive Healthcare Disease Management Program

Anyone know about the Positive Healthcare Disease Management Program by Medicaid?

I'm yet to meet my Nurse Case Manager, and I know nothing about the program or what it provides, and if it's even worth my time.

It is a managed care plan specifically for people on Medicaid with HIV (I'm going to guess that you're in FL, but other states have similar programs).  It gives you an extra layer of care, basically a nurse to coordinate everything with your doctors.  It is optional, and you can disenroll if you don't like it.  It doesn't cost you anything.  I would try it out to see if it works for you.  If it doesn't, you can dump it.

This is something new that Medicaid is trying.  I don't have any experience with it, and I don't know anybody who has used it.  So TAG, you're it!  If you decide to stick with it, let us know about your experience.

Thanks, I recently spoke with my "nurse case manager"....she was knowledgeable, really sweet, patient, and smart. I'll meet her sometime next week, and can't wait! I have a feeling, somehow, I will benefit from this program! I'll update y'all as things start to happen.

and yes you guessed right...I live in Florida  ;D
