2nd test results after starting Atripla, more good progress

I started in mid-September and became undetectable in a short 3 weeks.  Now, more good news.  I remain undetectable.  My CD4 went up to 615 (was 350).  And my percentage is 27% (I don't really understand this one.  Kidney function is normal and my cholesterol actually declined from an already low baseline so it appears I won't have those issues to deal with.  I've had no colds or flu or anything.

My only weight issue is that I seem to have to eat a lot more than before just to maintain my weight.  This has been a good thing over the holidays :)    I believe this is simply because I'm drinking less, hence need to eat more to get the same calories.

I'm taking it in the morning when I first get up and have no side effects from it.  I do think there is a subtle mood downer effect from it and when 1-a-day insentress/truvada comes along I'll likely try to switch to it.  But, overall, I have to say I'm really pleased with where I've gotten to in 6 months.  I saw my doc today and said "I don't feel sick" to which he replied "that's because you're not sick.  You have a chronic disease that is well controlled."  It was a good way to start the New Year and kiss the whole last crappy decade goodbye  :)

Congrats on you numbers rural.  :D

I'm going to be getting blood drawn tomorrow and see if my bff Atripla is still kicking HIV butt.

I hope with my good adherence and some luck that I'll have Atripla in my arsenal for years to come. It's been problem free for me.

hey ruralguy, great progress!! congrats!!  I'm coming up to one month on sustiva/truvada so this is good to read. by the way, I was also looking around over the past couple days for some info on the cd4%, found some good discussion threads here, and this link:

I was hoping I'd find a thread on here about Atripla and everyones numbers.
I'm kinda concerned about mine.  As you can see from my signature info below, I'm not having the same kind of rapid results as everyone else.  Should I be concerned about this?  I would've thought by now that I would be undetectable.  Don't get me wrong, I'm glad to see my CD4 and % numbers are going up, but I'm disappointed about my VL.
Is this ok?  Should I be concerned about anything?  Is there anything I can do to help further along the decline of my VL number to get to undetectable?

ruralguy, thanks....I did have my creatinine level for kidney function checked on Jan 8th, everything was fine. kidney impact seemed to be my doc's big focus for starting meds (sustiva / truvada).  will do cd4 and vl first week of Feb (more for my curiosity...I think my doc would be fine to wait another month or two).

Mikey, most of what I read on the forums is that 'trends' are the key indicator, and your vl, while not yet UD, is trending down. my doc has told me that it can be 3-6 months for the meds to settle in....which is why he's not been pushing the cd4 and vl tests yet (I do see many people get tested within weeks of starting meds). I've had no side effects to speak of, some skin problems have improved since I started the meds, and otherwise feeling fine, so he's confident that the meds are working.  I'm no expert, but looks like your trends are right and guessing you'll get to UD soon. clearly the meds are working for you and I don't think you have anything to worry about. I'm hoping for good numbers too for my next test, but as long as they are going the right way, that's fine.  Sure others with more experience will weigh in here about your concerns and other things you could do to help lower the vl. Anyway, good to see your post and good results, and hope you get to UD next test!!! keep us posted. esper

Hi mikey dont worry your numbers are great and all going in the right direction, cd4 going up , % going up and Viral load coming down, were all different some people take longer than others just stay adherent and remember take your meds the same time if possible and always one hour before or two hours after food with atripla, good luck.

Hi rural guy thats great news im feeling good as well , still thinking of taking my meds in the morning like you are as now i seem to have no side effects. I was also thinking off the switch to issentress truvada but not so sure now as both are good regimens and if im doing well why change , im not sure yet.

Hi All:
Just joined the forums --- diagnosed HIV+ in 09/08.  Started Atripla 12/4/09 --- so far not too bad.  Some stomach cramps and the required normal bowel movement ---- which I am having more of. 

Just got labs back from 12/29 --- cd4=540, 30%; vl = 60

Labs from October -- pre-Atripla were: cd4=473, 38%; vl=2,900

Labs from July 09 -- pre-Atripla were: cd4=520, 40%; vl=5,500

Should I be concerned about my falling cd4 %?  Only medical issues I have had since being diagnosed are:  slightly enlarged spleen; GERD -- no more chili for me; and get fatigued quicker then I used to.

Looking forward to being a part of the Forums to give and provide support.  Thanks for any feedback about above.

Hi all, first post to forum  :) Started Sustiva and Truvada 2 weeks ago after last results of CD4 334, 10%, VL 160,000 in late November. Had some mad dreams for first few nights (taking both at 10pm), and now just feel a little groggy in the mornings. Luckily no other side effects.
Have next VL test in February, hopefully will see a fall. Side effects are nowhere near as bad as I had feared and prepared myself for, so hopefully this combination will work for me for years to come. Will let you all know how I get on at next blood tests.

I switched my dose to the morning because my morning routine is so much more stable that my evening routine.   For others considering this here is what I learned...

- I need to take it when I get out of bed, first thing.  It seems I need to have it in my stomach about 30 minutes before I eat (regardless of what I eat) or I get that stoned feeling for about 2 hours during the morning.  The stoned feeling is not entirely unpleasent but you really cannot expect to do anything serious during that time....eg no major financial decisions, no big issues to deal with at work, etc.  But just 30 minutes separation between taking in and eating breakfast and I don't experience any side effects at all.

- I sleep a lot better than when I took it before bedtime.  I didn't realize it but apparently it interfered with the quality of my sleeping.  That morning hangover feeling disappeared quickly but I was not well rested.

- Emotionally, taking it first thing in the morning somehow relieves me of thinking about HIV at all for the whole day.  I was really surprised and stunned by my diagnosis and moving the dose to the morning lessens the mental burden of it. 

- I moved it ahead by only 2 hours each night until I got to take it in the morning....so there was 26 hours between doses for a few days instead of 24...my doc's advice.  The other option was to take one 10 hours early, then every 24 hours as normal. 

I know people react to this drug differently, "your mileage may vary" but I wanted to share the benefits I have perceived from a morning dose.

 Hey RURAL GUY as you probably know atripla has to be taken 1 hour before food or two hours after , im going to discuss with my doc about morning doses as i have no side effects nowadays even when im up for the toilet in the night (initially i felt like i was on a boat) but i dont feel i get quality sleep, to restless with the dreams i guess

I'd recommend you try it.  You can always move it back to the evening.

Just wanted to say that according to my doctor and also what I read on TheBody website.
It's recommended that people take Atripla on an empty stomach, to lessen side effects. But it can be taken on a full stomach, without any loss of effectiveness, if side effects are not an issue for the person on this regiment (probably not recommended for someone just starting Atripla). I can take it right after a dinner without any problems. Although this might not be the case for everyone... plus other issues, including quality of sleep could be a deciding factor. I guess everyone is different.

Well after 3 months on Sustiva / Truvada have gone undetectable, which for the test my hospital use is <10 copies / mL. Hopefully will be able to stay on this combination for years to come.

I was a little confused because the initial thread was regarding "starting Atripla" and you're regimen is a little different but I see it was started by another user.

In any event, I am glad all of these combo's seem to do the trick. Maybe Atripla contains the two drugs you are on. I should have checked that but anyway. Good for you.

In any event, I am glad all of these combo's seem to do the trick. Maybe Atripla contains the two drugs you are on. I should have checked that but anyway. Good for you.

Not sure who you're replying to but Atripla is Sustiva + Truvada.  Truvada is Viread and Emtriva.

So in reality Atripla is Sustiva+Viread+Emtriva or Tenofivir+Efavirenz+Emtricitabine.

Yes I realized this after I re-read all the components of Atripla. Like I said in my post, maybe Atripla contains these 2 meds but I did not see it initially. Oh well, I need to educate myself on the different names of all the drugs.  :) Thanks for the info Hellraiser.

My ID doctor actually gave me a giant poster sized HIV Medication picture guide with all the different meds on it and their generic names etc. when I started treatment.

The first thing my Partner said is "Are you going to hang that on the refrigerator?" No I laughed but I have used it as a quick reference when reading these posts.
