Losing ADAP assistance because we do not have AIDS...

First of all....I have been living successfully for over 20 years knowing that I am HIV+. I feel extremely lucky and thankful for for a long list of items....which I do not need to expand upon.

I live in ILLINOIS and I work part time...with limited insurance and thankfully and luckily my SUSTIVA and TRUVADA are covered by ADAP.

I have heard some chatter that some "powers that be " feel that ADAP assistance of HIV/AIDS medication should only be available to those living with AIDS.

My assumption that some person may make a decision that will take my ADAP assistance away...thus take my SUSTIVA and TRUVADA away from me frightens me.

I do not look forward to living my life without HIV/AIDS medication.....until my blood counts allow me to be labeled "Living with AIDS".

Has anyone heard of these type of discussions??

changes are being discussed not to change ADAP but to Social Security disability qualifications ("Social Security Disability: Will Recommended Qualification Changes Help or Hinder Access to Services?" by Tim Horn at http://blogs.poz.com/tim/archives/2010/09/social_security_disa.html). Access through ADAP is still based upon finances and having a prescription for HIV meds.

Although several state ADAP programs (9 states) have waiting lists and others are putting cost containment measures into place (see the current ADAP watch list http://nastad.org/InFocus/InfocusResultsDetails.aspx?infocus_id=355), the Ryan White Care Act was extended for another 4 yrs when President Obama signed it in 2009, so that it will continue to provide meds.

So no, they are not discussing 'only people that have AIDS' as being able to receive meds, but some states are basing access to meds upon (changing) financial requirements. In Illinois specifically, that state has already reduced the formulary and instituted a monthly expenditure cap; by 3/31/11 the state might lower the financial eligibility to 350% FPL (according to that ADAP watch list report)

Illinois is just 11 miles away so we get  Illinois news  several times a day.  Haven't seen or heard anything. That state is sure in some financial straights though,   but  you have to keep in mind, ADAP is federal funds that is just administered by the states. that is part of the reason the rules vary so much from state to state.

State funding is a minscule portion.  See page 8 and chart 13 on the report issued May 2010

• State funding accounted for $215.3 million, or 14% of
the ADAP budget in FY2009, a decrease of 34% over
FY2008.
• In FY2009, manufacturer rebates reached their highest
amount and percent of the national ADAP budget
in the programs history. Drug rebates accounted for
$497.6 million, or 31%, of the national ADAP budget
in FY2009. Rebates represent an increasingly critical
component of the ADAP budget, and drove overall
budget growth over the period, accounting for more
than 50% of growth between FY2008 and FY2009.
Drug rebates have risen from six percent of the
budget in FY1996 to 31% in FY2009. ADAPs must
actively seek drug rebates and, while not all ADAPs
do so (because of varying state drug purchasing
mechanisms), drug rebates accounted for a quarter
or more of the ADAP budget in 20 states.

Gee Granny, if you read your own link the state portion is "minuscule" because it decreased 20% from 2008 to the present.  So yeah, edfu is correct.

yes, the state portion is minuscule but NOT because that's how it's supposed to be. Over the last few years especially, some of the states have been funding ADAP less and less - so the federal matching funds have been less and less too. And every year more and more people apply to use ADAP

For example if a state funds $50 mill and the fed supplies $50 mill then that state ADAP has $100 mill
however when a state funds $25 mill and the fed supplies $25 mill then that state ADAP has   $50 mill
(this is just an example as I don't believe that it's a 50/50 match. does anyone know if there's an actual calculable match figure?)

but it shows an example then when the ADAP need doubles from one year to the next (because more people test poz, more people start meds, and more people need assistance) how extreme the deficit can suddenly be.

although the need for ADAP rose in SC this past year by hundreds of new clients, our state underfunded ADAP which will result in the US gov't lowering their contribution. With both state and federal dollars lowered not only can SC not take on new clients; but it must drop current clients.

exactly why I said the State portion is miniscule.  I wouldn't have said that if it was HUGE.  the figures  show it is only 13 or 14% of the total. there are some states that claim they have no money to contribute.  in many states there are more people that get their meds from Medicaid than from AIDS Drug Assistance Program ( ADAP).

It depends on who takes over the computer.  I haven't seen the female version of granny in quite a while.  Everything that has been posted recently under this screen name is definitely done by her husband David.

Yeah, I was quite sure that I recalled waiting lists in some states at some point in the 90's.
i knew there had been issues starting up the program (basically changing it from providing AZT to providing ARVs) but I hadn't realized that it had had waiting lists from very nearly the beginning.  It seems then that it's always been a troubled program without enough funds. That's tough since according to Kaiser (http://www.kff.org/hivaids/upload/1584_09.pdf) about 30% of HIV positive people use the program.

oh, and I recently learned that the NC ADAP financial eligibility is 150% FPL. Good grief! What's somebody making $15K (probably without insurance) supposed to do?? (maybe what our forum friend gymnut did, LOL, and just move to state without ADAP issues.)

"Drug assistance has grown since 1996 to become the largest component of the federal Ryan White program, which provides grants to states and localities. The drug program’s budget from all sources is now $1.6 billion, with Washington contributing about 55 percent, states offering 14 percent and drug company rebates accounting for 31 percent, according to the state AIDS directors.

"A confluence of factors has caused the strain. Enrollment has spiked during the recession, up 12 percent from June 2008 to June 2009, to about 169,000 people. The trend has probably accelerated since then. In Florida, monthly enrollments grew by a third between May 2009 and May 2010.

"A renewed emphasis on testing is also driving up caseloads, and federal treatment guidelines now recommend an earlier start to drug therapy. Because the drugs are so effective, people often stay on the rolls for extended periods.

"Meanwhile, federal financial support has stayed essentially flat, up barely 2 percent this year, while appropriations from state budgets have fallen 34 percent, according to the state AIDS directors. The drug companies increased their contribution by half, to nearly $500 million, but it is still not enough."
