What should I do after vomiting?
« on: July 18, 2013, 12:36:47 am »
Hello, I take Norvir, Reyataz, Kivexa daily, and those should be endeavored with food. But just 3-4 hours ago I suffered terrible vomiting, probably due to bad food. Currently I feel pretty weak and my belly is 100% empty. What should I do with my daily dose, as the time to take it has come?

If you can't eat, and might vomit, and are otherwise compliant with your dosing, then I would absolutely skip this dose and hope tha tyour next dose will arrive to find you in better spirits.

A missed dose now and then is not the end of the world.

OK, thanks a lot chap. Yes, I am very strict, and for some eight months that would be the first dose skipped.

Can you keep anything down?  If so have a piece of bread or a cracker or two and take your meds.
Pray God you can cope

It is great that you are diligent about your dosing! But if you are sick to your stomach, better to skip a dose than force yourself to take it, perhaps throw up a portion of it, and be half-dosed. THAT is when resistance can emerge. I really hope you feel better soon. Throwing up is the absolute worst.

Disagree. Better to miss a dose than to barf up half a dose. It's the half-dosing you need to worry about, not the occasional missed dose.

Surely a one-off, barfed-up half dose is no worse than a one-off completely missed dose? I'm not following your logic.

Besides, unless you're barfing up whole tablets that you can actually see and identify in your barf, then chances are good that they've already been pretty much absorbed or moved deeper into the gut before you've barfed.

I'm pretty sure is six of one, half dozen of the other. But given the choice, I would rather skip the dose than go through the barfing process (which would, I imagine, include the requisite meal that one takes with the drugs).

I recall taking first gen Viracept - or rather, attempting to take them. Huge, uncoated blue bastards would stick in my throat and I would either barf, or force myself to chug water until they dissolved (kinda) in my throat, leaving it burned for hours.

I did read somewhere that, if you keep your stuff down for twenty minutes, you have given it time to dissolve. But like I said, it's not just the whales, it's the water (to quote Star Trek IV).

OK, I get your logic now. Nobody likes to puke.

What confused me was your prior post about developing resistance - I should have also quoted that post I 'spose.

And yeah, I remember reading in several places where you're ok if you've had at least twenty minutes between taking your meds and puking. Hell if I can remember where I've read it though.

This does shed light on the fact that there are no treatment options currently available for someone unable to take medications orally.

Fuzeon is injectable however, one med does not make for an effective HIV therapy.

They are currently testing an injectable rilpivrine and the new integrate inhibitor (forgot its name).  Hopefully they will have success.  Otherwise, should anyone be laid up and unable to take your meds orally you could be up a creek.

Here is the link to that trial info:

My question is that they are testing these two meds together, but don't we need three meds?  Does anyone know if these two together would be sufficient, or if there is a third that could be combined for a monthly injection?

I absolutely didn't mean to imply that one half-dose (or one barfed dose) would lead to resistance, but a few weeks' worth of them stands that chance.

And I share your concern, Buginme, about the dearth of options for those unable to take medications orally. There really isn't much wriggle room in that department, is there?

On this combination, if you vomit more than 2 hrs after taking your meds then they are all in your blood system already.

Before that, possibly not in totality.

I agree with JK that missing a day cos of chucking will be no big deal. It's intermittent low dosing/no dosing that leads to resistance.

Thanks a lot for your answers... I was late an hour and a half, but suddenly felt better and took my "dose"... In the same context, I wonder one more thing: how long after taking the meds it's safe to take an enema? I read suggestions from 20 minutes to two hours, that after such period the meds are absorbed in my blood, or?

Unless the meds you take are in the form of suppositories, I can't see an enema making any difference to when you take your meds by mouth. Opposite ends of the digestive system on most people.
