CD4 drops almost 200

Just got lab results this morning and after seeing my CD4 increase from 179 in Mar 06 I have my first decline.

                OCT  08         Jul 08
CD4               397             580
CD4 %            23               26
VL             Undect.         Undect.

Since the viral load remains undetectable I don't think I need to be overly concerned (or should I be concerned), however I am not very happy with the drop.  I actually won't discuss these results with my dr until my appt next week.  I do have a question, do I wait till my next quarterly checkup for new labs or do them sooner in order to see if this is actually a blip or something more?

Well, yeah since you're still undetectable any doctor I'd think would say that these numbers are no cause to change med regimens or anything, and 397 in and of itself isn't particularly shoddy.  But also your % went down by 3 as well.  But I understand you're concern -- a drop of, say, 100 would have been easier to dismiss.

Bottom line though -- I've had this happen and then 3 months later it's all back to normal.  But since you're a rookie and this is the first time it's happened to you it's just natural that you'll lose some sleep.  If I were to post my lab numbers from the past 3 years you'd see that this same situation has happened to me around four times and every time it's rebounded. (I just checked)

Now, imagine the days of early treatment when I was diagnosed, when ALL they had to go by was the CD4 and % counts -- there was no VL testing yet.  Definitely used to freak me out then :)

Definitely ask your doc what he/she thinks, but with an undetectable VL, this is probably an aberration. Depending on the time of day you drew blood, and the lab it went to, variations of 100 or so can occur. Besides, your current regimen is doing the best any combo can do if you are undetectable, so switching now would not be logical.

I agree with Philly, but in this case, just to make sure you don't obsess too long; see if you can update a little earlier this time just to see if the trend continues.  Say two months from now, instead of 3 or 6.  It isn't unreasonable to ask for more labs than usual to track a blip.  

Remember, theses things do happen, and it is always a bit spookey to see on paper.

Well, mine tend to rise and drop at the same pace as yours. If your Vl is undetectable you should be fine.
But keep monitoring.
