Possible HIV exposure
« on: April 29, 2007, 04:27:30 pm »
I have a question that I am posting for a friend of mine. Here is the situation:

Thursday night my friend Matt met a guy named Jim and went back to Jim's hotel room. When it came to sex they talked about using a condom, Jim pulled a condom took it out of the wrapper. Jim topped Matt, he pulled out before he came. After he pulled out Matt realized that he never put the condom on.

Friday afternoon I took Jim back to his hotel room and we hit it off. I told him that I was HIV+ and he said no worries. Jim gave me a BJ, when I was cleaning up I noticed that Jim had a RX bottle that looked like Atripla (didn't turn it all the way around, but I was once on Atripla so I know what it looks like). I never said anything more about HIV to Jim.

Later Friday Matt I am talk about what happend. Matt was very worried about possible HIV exposure. Matt told me that he had some minor anal bleeding.

Saturday Matt and I went to the doctors office to talk to them about PEP medication. The doctor told us that it was a low risk exposure since Jim didnt cum inside Matt. And that the PEP didn't work more than 12 hours after exposure.

Jim has been feeling ok right now, he is running a small fever.

Please give me some feedback on this situation.

Damien, it is true that the earlier PEP is begun after a risky incident the better the chances for it to be effective. However, it's generally recommended that it must be started not more than 72 hours after a possible exposure.

So I disagree with the doctor who said it's a waste more than 12 hours after exposure.

I suggest your friend give serious thought to PEP. Even though ejaculation didn't take place there was some internal bleeding and precum may have been present. As far as you seeing a bottle which resembles one you used for ATRIPLA, that's in the domain of maybe rather than a certainty.

Given what you have reported I think doing PEP should be considered seriously. Your friend should at least get another medical opinion quickly.

Good luck to him.

Cheers,
