Sounds to me like a mild sunburn. SPF4 is next to nothing and SPF 15 is the recommended minimum blocking 94% of UVB rays. The Maldives this time of year is probably getting some pretty intense sunlight so SPF 15 is probably not enough. Here in San Diego I usually go with at least a 30(97%) or 45(98%). Going higher doesn't make a lot of difference.

Wikipedia has a pretty well documented article, go figure.

The other think I don't know about is if HIV meds increase the sensitivity of the skin to sunlight making it burn easier.  I have had psoriasis for many years and I know that some of those meds (mostly topicals) do increase photosensitivity.

Sometimes when I have been exposed to the heat, I will get a heat rash. Usually small red bumps but it goes away pretty quick if I cool down but usually doesn't itch. TB meds can make you sensitive to sunlight (it did for my BF) but I haven't heard about HIV meds causing it. I would have to agree that this is probably a sunburn or some sort of heat rash but could be something else. I don't know much about tropical settings but definately up the sunblock. Have fun and be safe.

Hi,

Just a thought, it could also be the type of sunscreen that you are using, that may be causing the rash. You may want to experiment and try another sunscreen product.

For myself, I use nothing less than a 30 Sunscreen on the waist down, everywhere else an SPF 50.
