Do I need PEP?
« on: August 12, 2013, 05:32:07 pm »
Hi there - this forum is so wonderful. Thank you for being here.

I'm in a nightmare of my own making. Was in an alcohol induced blur 48 hours ago, and hooked up, anonymously, with a guy I met cruising.

We jacked each other off, and then I topped him, unprotected, for about 30 seconds, then pulled out and jacked off to completion.

I'm pretty much a mess about the whole thing.

I will obviously be tested, in fact, I already have a physical schedule in about 12 weeks.  But do I need PEP?

I'm so appreciative of your advice. Again, thank you for being here.

Hi Just . You had a risk and you will need to test for HIV . The fact that you were the insertive partner for a short duration is in your favor , however a lower risk doesn't mean no risk .

Its up to you and your doctor if you want to pursue PEP and its best to start within 72 hours for it to be effective .

You can test as early as 6 weeks past the incident and again at 3 months to confirm the results . If you choose PEP it pushes the window period out to 6 weeks post PEP and again at 3 months to confirm the results .

Remember to get checked for all other stds , many of them can be present without symptoms for a long time and are much easer to get than HIV .

Thanks so much, Jeff.

I should've clarified that I did call my Doctor. He said, that the risk level was low enough that he wouldn't recommend the treatment (like you said, insertive partner, short duration) and that he would see me at my physical. But I was so skeptical that I wanted a second opinion.  This is the first (and only) time I have ever been in this position, so I have to admit to a huge level of ignorance here.

Thanks again.

Hi Jeff,

I wanted to follow up and let you know that I got a call from my Doctor this morning, and he did indeed start me on Truvada and ISENTRESS.  Just took my first dose.

He seemed convinced that the side effects would be mild...I'm hoping that's the case....I imagine the uneasiness I'm feeling in my stomach already us just nerves.  :P

I'm wondering if you've seen positive things on this forum once a person has started the regimen after an incident like mine?

Thanks again.

In the ten plus years I have been giving risk assessment on this forum,I have never seen someone test positive over a low-level insertive situation such as the one you described. It is as brief as what happens during a condom break, and I honestly think that PEP was overkill. Also, PEP really needs to be started within 24 hours of the event in order to be as effective as it can be. Though the window is 72 hours, the effectiveness drops off quite sharply after 24. Just my two cents on that.

I absolutely anticipate you will come through this without HIV. However, please note that PEP extends the testing window by ONE month.

Therefore the soonest you should test is six weeks past the date of your last pill. A confirmational test can be attained three months past the date of your last pill, or four months total.

"Many people, especially in the gay community, turn to oral sex as a safer alternative in the age of AIDS. And with HIV rates rising, people need to remember that oral sex is safer sex. It's a reasonable alternative."
