I was just curious how many people here have never had a genotype test done.

I know I haven't, but then, they weren't around when I started meds.

It seems like many have had this test done at least once. I'm just curious if everyone has had it.

mini had her's after 15mths of various "failed" cocktails, 5mths in the hospital, and 2 surgeries. I had no idea the test even existed until she was having it. If I would've known about it, I would've demanded it be done sooner.

Hi Mark

I had one done when I seroconverted in April 2002..this is how I found out I had the same rare K103N mutation as the index patient.

I had one done in 2002 after a VL blip 2x in a row. I was diagnosed poz in 1993, went on meds in 1995. I switched to PIs in 1996 and then the blips happened in late 2002. If the doc hadn't hesitantly mentioned a genotype I never would've known either. Glad I insisted after learning about it bec it turns out I was resistant to the PIs. been on Sustiva ever since, first with Zerit and Videx, now with Truvada.

I've had a couple done. I've get the results of the latest one sometime in January.

I don't think viral load tests were even around when I started the Crixivan trial (not sure about that, though).

You need a detectable viral load to take a genotype test, right?

Actually, it was that requirement that brought this to mind. Rich (Pozattitude) said he was going off meds for a month or so to allow his viral load to build up enough to enable him to have a GART done.

I would probably have to do the same thing because I have had an undetectable viral load since July 1996.

I just wondered how many others have never had the test done. I see now there are a number of us.

Oh, I think you're right. The viral load tests did not become standard of care until either late 1995 or early 1996, I think. I had a couple done in 1995, and I know one was still considered "experimental" because there was concern my insurance wouldn't pay for it. We did it anyway and the insurance never blinked an eye.

I wasn't on the Crix trials, but I was on Crix for 11 years. Does that count?

Possibly. The test is done routinely nowadays before people start meds to make sure they don't have any inherited resistance.

You just started meds, right? You might want to broach the subject with your doc to get his take on whether one is needed.

Hi Mark,

My partner Robert started Sustiva and Truvada which improved his numbers for about 6 months. GART was done when his numbers started to decline and was attributed to an acquired resistance to Sustiva. He then switched to Reyataz/Norvir with Truvada regimen. His numbers continue to climb up steadily.

Just before moving to SF our ID doctor ordered the test for me. This was to determine if we had the same strain, and if I were to start meds, we would have prior knowledge if I have any resistance to any of them. But the test was inconclusive due to not having enough viral load.

after going on and off meds several times through the years because of the side effects, my doctor did one in 2003 as we tried to find a regimen a)I could tolerate and b) that would work. Wouldn't you know out of the 20 something pills a day I was taking, the smallest, easiest to swallow, never-made-me-sick pill of Epivir was the one no longer working.

We eventually got it down to 10 pills a day now.

I've had 5 vl blips through this year, and every time the doctor threathens

these tests to determine which drug to use next; but thankfully every time the blip has leveled back out.

If I ever have two upward blips in a row, I've get the typing done for sure.

I don't know how I'd feel to going "detectable" on purpose though - it happens enough when I don't try LOL- but no, really, if I had been staying at an undetectable level for a while, I don't think I could go off meds to make my VL rise. Sorry but if it ain't broke, don't fix it.

I had both genotype and phenotype tests back when I had my initial VL and CD4 tests. I know my hubby has had a genotype, but I'm not sure about a phenotype.

Yes sir, I had one.

My initial labs were absolutely HORRIBLE (very low CD4 count and a viral load of about 500,000). I wanted to start meds immediately, but my doctor made me wait a week or ten days while she ran a genotype test. At the time, I was angry about having to wait for the test results. But now I'm glad that we did it, because I learned more about my treatment options.

i see my doc in jan for the second time. i will have him run the tests then. i haven't had a cd4 percent also. damn it, should the phenotype be done as well?

Mark, I've had two genotypes done, but I am currently waiting on a Phenotype's results. The doc wanted to run the pheno this time as it is more accurate. The last genotype I had done was a little over a year ago. As soon as my doc gets the results of the pheno back, I will be starting on a new regimen, which the doc said will most likely include Isentress.

First thing my Doctor did, after I was diagnosed. I feel blessed to have a very thorough doctor who is quite up on things. Not only that he sits and talks to me and has gotten to know me personally as well.

Considering that I never was able to achieve viral suppression until last year, though I've been on meds for almost 15 years, I've had quite a few done. There was also a lot of hand wringing by all of my doctors.

i'm on my 3rd genotype test, i seen the vampire on the 17th, she took 10 TUBES of blood !
& i'm still some what drained today ! the test is worth it !

When I first was diagnosed I had a genotype done with my first set of labs. It was a request of my doctor to know what meds would be wasted on me. Luckily I had no mutations so I am all good

When I started I didn't have the best of docs (meant well, but a little behind) and he didn't even offer the option (I'm not even sure the test was available as I was behind myself back then). In hindsight wish I had had known to inquire about it because my doc was not happy with how my first combo was taking too long to reach consistent undetectable so he switched me to a PI backbone.

Switched out of the PI since then (back to Sustiva which I feared I may have had some resistance to but new doc didn't think so and he was right) and it has worked just fine. Been undetectable for years on PI and now the NNRTI so my doc assures me it is unlikely I have any major resistance issues based on my response to the regimens I have been on which is fine with me.

I've never had one done and I probably won't until such time as I've been on meds for a while and start showing detectable. This is because the man from whom I aquired my virus (I know, it's a mouthful!) had one done when he started treatment and had no resistance to anything. We go to the same clinic.

We discussed this last year when I had my first ever clear downward trend. My doc ran the test they do nowdays to find out if you can take abacavir without running the risk of the allergic reaction some people get, but it was decided I didn't need geno/pheno testing.

Still not on meds after ten years poz - my downward trend reversed itself - and it looks like I'll get another few years med-free.

for hiv,probably around 8 genotypes, 8 or 9 phenotypes. what can i say, i'm special.

YAY! You're absolutely correct, Kate, I did have an HCV genotype done. I was a 3a.

It just became a standard of care in my country this year. I was lucky, because a pharma company was running a study about new infections, and my ID Doc got me on it. So they ran the genotype.

As I have had to deal with resistance a lot, I have had several genotypes done.
They showed that i am resistant to most..... but always had new ones available just in time.

I'm glad I'm not alone - I've never had one done.

I asked my doc and his feeling was that since less than 2% of his large HIV practice had resistance issues, he didn't think it was necessary.
