Hello All,

first pardon me for re-registering and starting a new thread my last post is more than 2 years old and I have simply forgotton my previous credentials.

Unfortunately I had a recent exposure risk with a female partner. (Broken condom) I was did a test at 4 weeks past exposure and I am now awaiting the results. I plan to do another at 13 weeks

I was fine until a few days ago I suddenly started having a severe pain in my right shoulder. I am not aware of having hurt my shoulder to cause this. The usual muscle pain treatments have not helped.

I know you guys have emphasized that symptoms mean nothing but after seeing this article (see link below) I am devastated. Is this a new strain?

Please give me your comments thanks

Here is the article  http://www.journals.uchicago.edu/CID/journal/issues/v34n8/011630/011630.html

Jersey, the contents of that Chicago article notwithstanding, the odds remain very much in your favor against transmission having occured during that single incident.

HIV is not an easy virus to transmit. It's significantly more difficult to accomplish from female to male. Given that you were protected until the condom broke that makes the possibility of transmission even less likely. And of course you are assuming the woman you were with is HIV+, which you don't know for sure.

The average time to seroconversion is 22 days. All but the smallest number will seroconvert within 4-6 weeks after an exposure to the virus. A negative at 4 weeks will certainly be reassuring. You have the right idea about getting re-tested at 13 weeks.

Discuss your shoulder problem with your doctor. I think you're way jumping the gun at this point to be assuming it's ARS-related, although I can understand how your mind grabbed on to that one.

I still expect you will likely come out of this ok. Keep us posted on the 4 week result. In the meantime while you're waiting keep productively busy. It will help the waiting time to pass more quickly than you may imagine possible at this time.

Cheers,
Andy Velez

Jersey

I'm impressed you dug up those two case reports. As the authors point out, shoulder girdle problems are an unusual manifestation of HIV disease. Whereas musculoskeletal (including shoulder) problems are the most common presenting complaint if you are a general practitioner. So the chances are you just have a problem with your shoulder, and you've attached your fear of HIV to it.

In the 19th century syphilis used to be called "the great masquerader". It could basically present with anything. The "great masquerader" of the 20th and 21st centuries is HIV - it can also present with virtually anything, if you look hard enough, which is why you can only diagnose it with a test.

Andy is totally right about your low level of risk from this encounter. Stay busy and keep up posted.

All the best,

Matt.

Andy and Matt,

Thank you for the words of wisdom.

My shoulder pain got significantly worse yesterday and I ended up at the ER. In the end I was diagnosed with "Bursitis" something I had never heard of before. The medication they gave me has reduced the pain so far. They didn't even ask about my sexual history and I didn't mention it.

This morning I received the results of my first test. It was negative so I am taking that as a good sign considering that it was done exactly 40 days after my "incident". I will test again at 91 days.

Once again, Thank you very much for your advice, you have certainly helped me to calm down.

Given that the average time to seroconversion is 22 days and all but the smallest number of those who are going to seroconvert do so within 4-6 weeks after an exposure to HIV, a negative at nearly 6 weeks is very encouraging.

Taking all of the factors into consideration in relation to your broken condom incident I expect you to continue to test negative at 13 weeks.

Cheers,
Andy Velez
