I recently topped a transgendered woman in which one condom broke after another mostly like due to a lack of lube I didnt ejaculate nor did I see any visible blood on my penis, according to an HIV testing clinic coordinator these are factors in transmission. The  transgendered woman assured me she was recently tested though this landed on deaf ears considering this was sex with a stranger. The following day I researched options and under the suggestion of an on-call ID physician I seek PEP but this initially  fell through because I wasnt able to get blood work at an ER nor was I able to see an HIV specialist due to dealing with the wrong secretary. Later that day I was able to make an appointment with an ID specialist for the next day I was administered combivir under the 72 hr window. I know now all I can do is test myself at the 6 month mark for a definitive result

Aside from all of this I recognize this decision I made was not a mistake but a failure I have been very stressed and uneasy to be frank Im not very optimistic I recently discussed my situation to an acquaintance who then told me I should have been prescribed Truvada instead   

I know this might be an absurd question but does my future look gloomy?

To make more matters more complicated one day I decided to masturbate to relieve some anxiety, I flushed my semen but did not flush the urine that followed, a few minutes later someone else used the bathroom.  I hope I did not put someone in danger

You've got several things mixed up. You can get a reliable result by testing a 3 months after the completion of PEP. Not 6 months.

Even if your semen, blood and any other bodily fluids were all over the toilet after you used it, that would not have been a risk to the next user. HIV is not passed in that manner. It's a fragile virus and is not passed on environmental surfaces, by touch, kissing, nicks during a haircut and other means that people worry about needlessly.

Your risk from the broken condoms is a low level one. HIV is not easy to transmit sexually. It's significantly less risky for the insertive partner. Odds are you are going to test negative.

You ought to read our lesson on transmission and get up to speed on the subject. For now mainly you just need to know that condoms provide very effective protection against the only confirmed sexual risks: unprotected vaginal and anal intercourse. Make sure your condoms are fresh, fit you properly and that you use sufficient amounts of water-based lube.

I'm betting you come out of this ok.

Mister,

As we've told you, you are highly unlikely to end up positive over these incidents, with or without PEP. Stop freaking yourself out with every little twinge or changes in the appearance of your skin.

Get busy with other things in your life and the waiting period until you can test will go much quicker.

We're not here to hold your hand every time you get a ache or bump or whatever. If you're concerned about any physical stuff, go see a doctor.

No, it is not. That is not an HIV specific occurence. You also need to know that neither the presence nor the absence of symptoms will ever tell you anything accurately about your HIV status.

If there has been a genuine risk then only an HIV test at 13 weeks can be depended upon for a conclusive negative result.

Well I am back again, I know too soon. Last night I received covered oral sex, while the condom remained intact and I gripped at the band of the condom while receiving, I was cleaning up and  I noticed a small abrasion on my scrotum Im sure I caused it while scrubbing in the shower prior to the encounter. The oral sex was mainly on my penis and a few kisses on my chest and groin. My question is this did I comprise my HIV/STD testing schedule, was there a possibility of transmission of infection from both perspectives? Ive received the HEP vaccination several times and it didnt seem the wound was bleeding. I dont plan to have sex(or oral again)until I get conclusive results well with the exception of covered frottage

Mr,

Getting a blowjob, with or without a condom, is not a risk for hiv infection and neither is kissing or being kissed.

Not only is saliva not infectious, but it also contains over a dozen different proteins and enzymes that damage hiv and render it unable to infect.

You have not had a risk so this has no bearing over your previous testing window. I'm still fully expecting you to test negative.

i went to get tested for syphilis yesterday,i was also given a blood rapid test with a nat test as well

these are standard test that the clinic administers i did not request them since its too soon for antibodies
the exposure was 5/15/11 and my PEP was completed on 6/15/11

should I disregard the negative rapid test result and the upcoming nat result?

We have repeatedly told you that you were highly unlikely to end up positive as the insertive partner when a condom breaks during intercourse, with or without PEP. Your negative results are not going to change.

Update

My syphilis RPR results were nonreactive, HIV-1/2 + EIA HIV-1 RNA were also nonreactive

Well it seems I can assume that  I didnt contract syphilis

Mr,

You are highly unlikely to test positive as the insertive partner concerning a condom break. You also took PEP, which while unwarranted, pretty much guarantees that you are going to test negative in September. Your negative hiv results are not going to change.

Do not expect to use this forum to continually wring your hands over this situation. You can be given a Time Out whether or not you are in a window period. Please consider yourself warned!!!

And having done it 7 days short of 3 months are you going to fret about that negative result.

You're HIV negative. Be glad, cut out the drama and get on with your life.
