They are looking for a 'NO' there if you are negative, I believe.  It is awkwardly worded.

In the past five (5) years, has any person listed on this enrollment form been diagnosed by a health care provider, received treatment (including prescription medications) or been hospitalized FOR any of the following conditions or diseases?

1. Immune disorder (AIDS/ARC)
2. ...
3. ...

(the FOR instead of WITH in the question is making me confused at to i answer YES or NO though i took the test and am negative).

Isnt Testing considerded a part of being diagnosed FOR a particular condition?

How do I answer this???

Very Confused

Help Appreciated

ghj,

I think they used "for" instead of "with" because the last part of the question is about being treated or hospitalized. It is more common to say you've been treated "for" something or in hospital "for" something than "with" something. It would have been better for them to word it as:

In the past five (5) years, has any person listed on this enrollment form been diagnosed by a health care provider with, or received treatment (including prescription medications) or been hospitalized FOR any of the following conditions or diseases?

But not everyone pays close attention to grammar or sentence structure.

But anyway, as you have not been diagnosed with hiv, then you can answer NO to their question.

(edited because somehow I had the impression this was part of an employment application - not insurance. Sorry! I removed the irrelevant bits.)

Is this life insurance by any chance?  Anyhow, basically all they want to know is have you been diagnosed and have you had treatment for that or a hospitilization.  Testing does not count--if it is negative, then it doesn't qualify.  They just want to weed out the positives, because if this is life insurance you are applying for, it is very rare to find a company that will even consider covering an HIV positive individual.  But you are negative, so no need to tell them you have even been tested.  If you have been tested by a health care provider and that test was positive, it counts for a diagnosis.  It is not good to lie, because insurance companies somehow usually find out and that is grounds for termination of coverage if you have been found to lie on the application.

Why not give the insurance company a call. Tell them even though you are negative, you are confused by the wording and want to be sure you are answering accurately.

You might even be able to ask questions without identifying yourself.

The convoluted syntax of their forms notwithstanding, just try to keep it as simple as possible.

Cheers,
