Normal Lamivudine dosage denied by insurance?

My partner was originally prescribed lamivudine 150mg once a day while in the hospital to see how it effected his kidneys. It was bumped up to twice a day since everything was going well. That scrip ran out yesterday, and when trying to get the 150mg x2 filled, insurance denied it saying his "health plan requires quantity limits for this drug."

What is the deal here? I thought 150mg x2 was the standard dosage for lamivudine? His ID's office is already fighting with them on this but it could be a week if/when this is resolved. Has anyone else had a problem similar to this?

Hi,..

It is the standard dose. Each presciption came with 60 pills in the bottle. Pills are to be taken 12 hours apart. I was started on Lamivudine, back in 2003 , and still on it, in the form of Epzicom.

What other medications were prescribed for him. He needs two more to add to a regimen.

Current Meds ; Viramune / Epzicom Eliquis, Diltiazem. Pravastatin 80mg, Ezetimibe. UPDATED 2/18/24
 Tested positive in 1985,.. In October of 2003, My t-cell count was 16, Viral load was over 500,000, Percentage at that time was 5%. I started on  HAART on October 24th, 2003.

 UPDATED: As of April, 2nd 2024,Viral load Undetectable.
CD 4 @593 /  CD4 % @ 18 %

Lymphocytes,total-3305 (within range)

cd4/cd8 ratio -0.31

cd8 %-57

Same thing happened to me when I switched insurers and was taking Bactrim daily.  The latter said Bactrim was quantity-limited.  The pharmacy contacted the physicians office who submitted a prior authorization form and the problem was rectified.  It took a few days to be resolved.

This information is very helpful to me as I will switch to Humana.  I want to confirm if they also have a "quantity limited" prescription restriction.

Got Insurance list of medicines and saw Specialty and Quantity Limited categories for some of my medicines.  A few were not listed.  Trying to get more information on this and what it will mean to me cost wise.
