my risk and PEP treatment
« on: September 03, 2013, 03:48:24 pm »
Hello. I first want to say that I really am thankful for the exceptional work that goes on this site. What an amazing resource.

Onto my question and my panic...

Two days ago I engaged in risky behavior with someone who I know, but certainly not well. I was the insertive top, and what started as frottage ended with me inside him for about 2 minutes before I put a condom on. Very, very stupid. The person I engaged with said he was HIV negative but of course, that's most likely meaningless.

So today, after weighing my options, decided I should go on PEP. I just started this morning. My doctor gave me Combivir and Kaletra. But I'm now reading that Truvada is preferred. Should I try and get on Truvada even though I've already started this? If I'm going to get on it, I need to do it quickly and just want to see if Truvada is preferred b/c of tolerance or effectiveness. And also, if it seems as though I'm doing the right thing with PEP in the first place.

Thank you so much!

Combivir and Kaletra is an effective pep treatment as any and since you are on it I would continue with it .

PEP will push your testing window period out to 6 weeks post pep and again at 3 months to confirm the results .

Thanks, jeff! Glad to here it's just as effective. Is there anyone who can assess my risk from this potential exposure? How effective is PEP within 72 hours? Thank you!

There are too many variables to give odds on exposure but the fact you were the top for a brief duration is in your favor . PEP is very effective ... You should expect to come out of this HIV negative . I have never seen a case like yours end up positive for HIV and I fully expect you to go on and test negative .

Well, I wanted to avoid going into full-blown panic mode but here I am - in full-blown panic mode. I'm still on the PEP, which I can tolerate. And while I know the window period is 72 hours, I started it at about the 60 hour mark. Everything I've read said that earlier is better, and that the closer you get to the 72-hour mark, the more ineffective it is. I don't understand how on days 1 and 2, I didn't think it was that big of an exposure and then woke up on the 3rd day panicked, but I did. What is the likelihood this will not go my way? Again, it was about 1 or 2 minutes of insertive sex with a partner who said he's negative, but who really knows. Thank you! I will stop after this.

The risk for the insertive partner is significantly less than for the receptive partner during unprotected intercourse. So that's in your favor. And we're talking about a single relatively brief incident. Also in your favor.

You're just going to have to be patient and wait it out until you can test at 6 weeks after completing PEP. All things considered I expect you to come out of this ok.
