What kind of doc do you see?
« on: November 22, 2006, 03:39:57 am »
Reading some of the other messages made me realize that many people on here go to an infectious disease specialist. My doctor is a family and sports medicine specialist who knows a lot about HIV. I've been with him for about 11 years and I think he sees a limited number of HIV patients. I haven't seen an infectious disease specialist in all that time and have not felt the need to. If my PCP doesn't know the answer to an HIV issue he contacts the specialists at Wake Forest or Duke and gets the answer.

My partner sees a infectious disease specialist and complains how difficult it is to get an appointment and how little face time they have. One thing I love about my PCP is I can call his office with a problem and his receptionist's typical reply is "can you be here in an hour". My PCP always tells me his latest dirty joke, we gossip, he checks me out physically and asks the usual diagnostic questions.

I'm curious as to what kind of physicians other people see and if they are satisfied.

I go to an I.D. doc and I love him. Awhile back he started as my primary and I would have to drive out of the city to go see him. But then he started doing a clinic, so he suggested I see him there since it was closer for me. My question is how much face time do you expect to get? Doctors do have other patients, just saying. I usually spend about 20 minutes with him depending if I'm asking questions. I actually enjoy the visits and at the end he gives me a hug. I also feel that I'm at an advantage because my primary doctor has worked in his office in the past so there is great communication between the 2.

In the past I have asked my primary doc some questions concerning my diabetes and hiv clashing. The things he could asnswer he would but is also quick to tell me that he does not know a lot in regards to hiv. He does share some of the things he does know and what he doesn't know, he usually refers me to my id doc. I didn't find my docs easily but I am glad I have them now. I hate doctors that treat you like a statistic.

When I was diagnosed earlier this year, my family doctor - who I've had for almost 20 years - referred me to the local ID specialist. After my first meeting with him, he suggested, for simplicity, that he become my primary care physician. His plea made sense, given my bout with PCP, the pneumonia variety, and the lack of specialization that my family doctor had. However, now that I feel more educated and in control, I'm gravitating back toward my family doctor for the relationship and for the ease of making appointments - I can usually get in on the same day if I call in the morning with a matter of some urgence, whereas with the ID doc, it's a week or more wait. Fortunately, they're both good people who care about me so I will likely spend the time fostering both relationships.

I think that, much like in the business world, it pays to have connections. I can't predict my medical future with absolute clarity, but having two strong relationships can't be anything but beneficial should a need arise for which either is not immediately suited.

Oh, and one to shrink my head twice a month.  ;D

I have been weaving back and forth.  I have been to an hiv specialist only and stayed as long as I felt that he was taking into account the whole body.  When that wasn't the case, I would add an internal medicine doctor to balance the picture, especially if the i.d. appointments were brief.  On the Cape, I was seeing one or another male n.p. who was part of the hiv 'team.'  In Hyannis, I am healthcare connected to the i.d. end of the hospital, find them- all women except the unseen male head of the practice- very good about access and time but....  My but is that I am most relaxed with a male doctor and any doctors accepting any patients on the Cape are rare.  Now I did find an occupational physician who, oddly, did his internship at St. Vincent's infectious disease center in NYC where I am from, who is an internist so I think that I might see if I can get an appointment with him and make use of his background even while maintaining my relationship with the i.d. end of the same hospital.  Win

I see an internist (internal medicine specialist).

He is quite good and very caring. He also is the only doctor available in the area unless I cross state lines. In state, the nearest doctor aside from the local one is 190 miles away - one way.

My problem is the local doc is planning to retire in June. UGH! But, he has said he would keep his HIV patients and run a clinic for them if a qualified, willing doctor cannot be found to replace him.

There is a doctor at the local hospital who is knowledgeable about HIV. In fact, he was my doctor until he decided to join the hospital staff. So, at least there is someone at the hospital who knows what he is doing. That can be a rarity in rural areas.

HUGS,

Mark

I go to what's basically a Ryan White clinic, but it's exceptionally nice and small , ina beautiful rural town. ID doc is female, young, and is only there Tues and Thurs. I only go for labs and immunizations. I get all the face time I need, sometimes 45 minutes.

I see a derm. Once I told her( derm's PA) I was positive, I get any appointment I want because I'm only there 5 minutes.

SO , knock wood, I haven't had to see any other doc., but I believe the clinic has added a NP to help out with other issues, who may be there 5 days.

Mine is an I.D. Specialist, I've been with him 18 years.   Problem:  he's in Atlanta, and I'm in North Alabama, 2.5 hours away.    I'm going to be switching my medical care to UAB 1917 Clinic in Birmingham over the next 2 months, and you have no idea how uneasy this has made me....I hate leaving my doctor...but he will be retiring soon, and now, while I'm in fairly good shape, is as good a time as any to move on.   I'll post more about all this as it occurs.

Ford, your doctor sounds great!   I'd stick with him, bud.

hugs,

Alan

My PCP's calling card says "Infectious Disease &Travel Medicine".  Judging by his waiting room, I would guess maybe 20% of his practice is HIV/AIDS (of course, I could be way off there...it could be higher.)
I also see a rheumatologist regularly.

I am lucky that my GP specialty is Internal Medicine/Hiv Medicine so I go to  him for everything and am lucky to have him in my life.  I actually look forward to going to him since he always seems to put my mind at ease.  He has a large number of HIV patients in his practice but I still get all the face time I could need.

I go to an ID specialist for everything.  I am blessed in that the hospital he works is only 4 blocks from my office.  I know if I didn't have someone on top of the lastest in HIV treatment things wouldn't be so good.  He's willing to tweak my regimen around to reduce toxicity, while playing it safe to keep suppression.  From some of the treatment advice I hear about I feel fortunate.  As most of us know, this is not a disease to fool around with and a mistake in treatment can be fatally costly.  I'd say starting out with a GP is totally fine but once there's resistance, you better bring out the big guns.  At the very least, I'd supplement advice from a GP w/ inquiries on here or from Dr Gallant at Johns Hopkins.

I see both...my doc is an internist, and I have my ID doc. My PCP prescribes my meds now, and he sends my labs and scripts to my specialist. So, they are in constant contact...I like to think of them as my pit crew! I wanted to have a good solid medical team...just in case!

I feel good about it now...they treat me well!

None or the duty doc for HIV, A&E doc (yuk!) or the vet (mostly the vet) for primary care ... It seems impossible to register with a GP in Brighton (why?) and at clinic I see the nurse, and various people write to me about taking my tabs properly.

Bit more seriously, though, clinic is well worked out for stable people on / off treatment - nurse for bloods, results by phone (er, or sometimes not til you ring really if nothing's wrong), option to see a doc if you need, collect drugs from the pharmacy, coffee at a nice Italian place after.  Shame, miss going to clinic more sometimes, can be kinda cruisy and I see my friends.

The one doctor in charge of my over-all-care is an HIV specialist and gay. He is well known in northern California as one of the best. My cardiologist is also very knowledgeable with HIV issues. My Urologist and prostate doctor, also is well versed in HIV. And finally my dentist is knowledgeable in HIV, as well he’s gay and treats mostly HIV+ people.

So I pretty much have everything covered as far as health care goes. And I do make the extra effort to make sure that they all know what’s going on with me health wise.

Oh I almost forgot about my Eye doctor. He’s not gay but is around my age(Yes, He’s still in practicing at that age.) but he has had a number of friends that have died from AIDS over the years so he caters mainly to the HIV+ community.

One of the most important thing for me with doctors is for myself to be completely open and honest with them. They must be the same way with me and they usually are.

I see the same GP I've been seeing for about 16 years, long before I was HIV. He has some sexual diseases diploma and other letters after his title.

The practice is predominatly used by gaymen and there are a lot of HIV +ve patients. My doc happens to be gay, but that was never a prerequisite. He's been there for years, so knows his stuff. Best to see him early morning though, as he can get a bit dithery later in the day  :D

They are not allowed to give results over the phone in Aus, but I don't bother making another appointment to get them. If something is astray, he calls and tells me to come in. Always hate that call, as it means something is up. Otherwise, I wait 3 months for my next scheduled visit to pick up my last lab results.

I get 20 minutes with him, then up to the nurse who takes my blood and other measurements for the trial I'm on. Oh, and she gives me three months worth of pills.

I also visit him for any other ailments I may have and can always get in on the day, if urgent, or within 24-48 hours otherwise. I like just having 1 doctor and one place to visit.

Be forgiving, be grateful, be optimistic

I see an ID Nurse Practioner in a university hospital.  I am extremely satisfied with my level of care... and going to a uni means that they kind of guinea pig me a little bit... in that I get tests for things other people normally wouldn't get and things like that.

They all manage to keep up with my ever-changing hair color...

And I also, for the first time in my life, don't lie to the medical professional, because they're so extremely non-judgemental.

My Dr. is an internist at a big public hospital. There is a specialist in HIV in the same hospital but I have been assigned to him by the NHS. He is chief of residents and head of department at the university. At the beginning he seemed very weird...because he is. I didnīt trust him very much at the beginning because he treats you af if HIV was nothing to worry about, so I thought he was careless. But he sees HIV patients on Mondays and Thursdays. So the other day I met a young woman that has been treated by him for 22 years. She was there with her daughter who was at  lesat 20 years old. She is healthy as all the rest of the patients in the waiting room, so that makes me feel safe. The pharmacy department is great. They take you appart to an office to teach you everything about how to take your meds and the importance of being adherent.
I also see the same GP since I was a child. An ex military Dr. he should be retired by now and he spends a long time with me but telling me things about his family and grandsons...It is like going to visit an old friend to discuss everything except health issues. But the truth is I donīt have any health issues yet.
From time to time I also visit a GP practitioner from the social security at the local clinic. She is very nice and was the one who arranged everything for me at the hospital after my diagnosis. My mother also goes to her to have her meds for thyroids and osteoporosis prescribed free.

All my HIV issues I go to my ID doc he's given me his mobile and e-mail address if I ever need to get hold of him which I thought was a nice touch.  I havent disclosed to my general doctor because the hospital my ID doctor is based at has a HIV drop in clinic so if theres ever anything i'm worried may be HIV related I can call in on them.

I have a ID doc who handles my HIV/Sexual health care and a GP who handles everything else.  I think my ID doc is a big wig...she is (or used to be) head of some British doctor's association.  And I always see her name in the papers commenting on the state of sexual health care in the UK.  She's a good doctor.  Very thorough.  Not the most gentle doctor....very straight to the point, slightly rough around the edges.  She's always asking me about my sex life (how many guys, usage of condoms or not, all poz guys or some negs, oral sex, masturbation, blah, blah, blah) sometimes that can get on my nerves.  She's also big on the unusual sexual diseases, so she likes to tell me about those that going around Europe at the moment.  I'm not as comfortable talking to her about my sex life as I was with my previous doctors....it feels a bit like talking to my mother about sex (gay sex no less).

I love the nurses at my clinic.  They are very funny.  Down to earth.  And they know their stuff.  I get to chit-chat with them and I don't even mind them doing their torture sexual health screenings (required to do it 1-2 a year).

Actually all the nurses I have had in my HIV practices (Houston, NYC, & now) have been great.  That's one thing I will say about HIV care, we sure do get some good nurses.

I got hooked up with Dr. Ben out of Denver through my GP in Aspen.  Dr. Ben has spent literally hours with me & Will over this past year and I love him to death!  We are going to hit the slopes together if everything works out..

Dr. Young is an attending physician at Denver ID Consultants at Rose Medical Center in Denver.
The much-traveled Dr. Young is a long-term resident of Colorado, but was raised in New York and San Diego. Dr. Young began his graduate studies at the University of Colorado in Boulder, where he received a Ph.D. in molecular, cellular and developmental biology working in the laboratory of Nobel laureate Thomas Cech. He went on to earn his medical doctorate in 1992 from the University of Colorado School of Medicine and completed his post-graduate training in internal medicine and infectious diseases at the University of Colorado Health Sciences Center.

The author of numerous journal articles, reviews and studies relating to HIV and its treatment, Dr. Young's research was most recently presented at the 2005 European AIDS Conference in Dublin, Ireland and the 3rd International AIDS Society Conference in Rio de Janiero, Brazil. He is a principal investigator with the HIV Outpatient Study, a massive ongoing analysis of the health of HIV-positive people throughout the U.S. A member of the HIV Medicine Association and the American Academy of HIV Medicine, Dr. Young is also actively involved in educating healthcare professionals and community groups throughout the world on HIV-related subjects.

My ID Drs has become my PCP.

I just totalled up the numbers so far.  Twice as many of us (18) see ID Drs., 7 of us still go to our GP's and 5 of us see both.

Not sure about Newt.  I think he takes care of himself.

I'm kind of lucky as I go to a clinic 2 blocks away that is an HIV specialty center. So, one, there is no discomfort about other patients there who are not poz. Also my Doctor is the lead on the Pfizer study I am on. He is not the "hug you at every visit" type, but I respect him and feel secure in his care. I have on a few occaisons seen another doctor in the center when mine was away and that was fine. I also have a coordinator of the study that I can contact if I need information or to get something done. There is a Doctor on call 24/7 so if there is an emergency, or a complication of some sort, I can call and reach someone. It helps that for the most part, I am well. I only go once every 3 months at this point.
