what can i expect?

hi everyone,

last few weeks, since i was tested positive most of the day i am spending in front of my computer and reading this forum for hours. everyone arround me is telling me stop doing this but i just can't. i was thinking should i write something here since i am very conscious about my bad english but i will try.
i was tested positive on march 10 2014. few days after i went to get my adap since i am foreigner. i went to do a blood work and my result was VL 7401 and CD4 487
i know it s not bad and it s not so great.
i didn t get any other diseases ( hep a b c, siphilis, gonorrea or similar ), i never had any problems with health..yes cold here and there, felt tired sometimes but i work a lot so it might by that i was just exhausted. started with therapy 2 days ago, Norvir, Prezista, Truvada. FIrst 2 days no side effects.
i am writing here cause of one thing?
1.i know it s very individual but how long take that adjustment period ? how long i will need to accept fully that my life is going in some new, different way?
cause right now i feel like i am in a limbo. i don t want to see anyone exept people that knows. my 2 poz friends, my 2 neg friends that knows my issue, my boyfriend ( thanks god he is neg ) who is very supportive, my family who is supportive too. i am avoiding everyone else.
2.i see here people living with hiv for 20 and more years but i am curious with how many complications ( in terms of OI ) one  has to face every day, year? how often OI comes ? what can i expect with my numbers? how big are chances that i will get KS or Pneumonia. i am trying to eat better now, i am planning to start exercise, quit smoking but i am just affraid of what will bring tomorrow.

doctor, my poz friends are keeping telling me ull be fine, don t worry u will live a long life but somehow fear inside of me is not letting me live and enjoy life. i rather stay home and read this forum for hours and thinking when will my ks show up.

one more thing. i told my doctor that i want to start therapy asap. he said ok and he said he has combo for me but for that combo i need to wait 2 weeks until the resistance test is done. i said that i would like to start it asap and he said..ok start with truvada, norvir, prezista. do u think he is shallow putting me on meds without resistance test? i don t know since the hospital is mount sinai on 25th street and they deal only with hiv patients..i assume they have lots of experience.

As you said, adjusting varies for each individual. I can only share my brief experience so far.

Getting used to the medication is the easier part. Took me about a month for the meds. Of course, I got a 7-day pill box to keep track of my pills. I also set a reminder on Intelecare to remember taking it.

Mentally, it's very different from person to person. When I was first diagnosed, it occupied my mind constantly. I kept myself busy with work and sleep to stop thinking about it. When I did get stuck thinking about it, I hit these forums and read everything that I could find about the disease.

It took me at least 3 months to stop thinking about it all the time. And that was with a psychologist helping me.

Socially, I can finally claim to act normal after 4-5 months. Other than treating doctors, I have only told my psychologist and a close family member. You are lucky to have that many people know and support you.

With your numbers, I'd expect your doctor to tell you: very low risk for OI's. Unless you have already had an AIDS-defining OI, your chances for one now are very low.

Still, you should talk to your treating doctor about your case.

It's good that you are making these changes in your lifestyle. You are investing in yourself for the long haul.

Remember to take each day at a time.

Far from it. Protease inhibitors (PI's) (on top of background therapy) have the highest barrier to resistance of all ART classes. Without knowing what drug resistance you have, your doc is actually being ultra-conservative.

Your regimen is actually a first line recommended by guidelines. My doc put me on the same regimen until my genotyping came back.

Once you get yours, your doc can always switch you and simplify your combination. He tried to switch me to Stribild, but I got a rash. Going through a trial with Truvada/Tivicay. No problems so far.

Hey muratti, welcome.
You know, for a recent diagnosis, seems to me you are surrounded by a very comprehensive support group, and congrats for that.  I think you will get out of limbo pretty quickly because you are being matter of fact about it. And the people around you knowing will just help it become normal again.

I don't think you should be worrying about how many OI's you are going to get. You are already on treatment, and your immune system is in good shape starting. Really you will probably have no troubles with OIs .

I don't want to discourage anyone who says they are going to exercise, quit smoking, eat better.  But listen to this - a RADICAL change of lifestyle is a lot to manage. But you already have one big thing to manage, for the moment.  Please don't feel like its necessary for your health to radically change everything and instantly.  The HIV treatment is the thing that deals with HIV infection.  You don't have to have a perfect diet or exercise plan to completely control an HIV infection.  Is that clear? Its the truth!

Nutrition, exercise, quitting smoking - those are all good things for ANYONE's health, not just for HIV+ people.  Really, if it sounds stressful to take everything on at once, relax, breathe and go about your life as normal as possible. Take baby steps on the lifestyle changes and do stuff when you are ready to, and calm about it.

I have one more question. do u think it can be harmfull with my cd4 487 and viral load 7400 the cold i have now. i started my meds 5 days ago but i don t know does this cold can affect me in a bad way?
it s not cold actually but stuffed nose in the morning and a bit of blood every morning when i blow my nose. it could be also from dry air in my apt but i am just affraid of everything now.

We get sick and its not the end of the world or doomsday . When you are HIV positive its important to listen to your body and part of that is knowing when its telling you you have the sniffles and not pneumonia .

Having a cold is not HIV related so just watch out for fever or something you may need to see a doctor for . Colds need chicken soup and not antivirals .

Just continue to work with your doctor and decide when its best to start treatment ... but a cold or sniffles is not a thing to worry over with your lab numbers .

morning jeff.
i did started treatment 5 days ago. truvada norvir prezista.
i am affraid of pneumonia too cause i smoke too. i have to quit i know but it won t happen over the night cause i was heavy smoker.
is it possible to get pneumonia even if u are on treatment and the cd4 are in high 400 like mine?

Tough Love ... Its natural for the newly infected to worry over health issues but you must begin to realize that worrying over something that has not happened is not helpful .

You should live your life like you did before HIV and that means if you get sick see your doctor when you have a fever or something is obviously not improving over time .

You are not in danger of PCP pneumonia with your lab numbers but you can still get sick and it has NOTHING to do with HIV . What you are experiencing is anxiety over your diagnosis and maybe a sniffle . If you continue to worry like this please tell your doctor and ask to see a therapist to help you transition into a person who can live well despite HIV .

Its good that you are posting about your anxiety, maybe it will help you separate fears from facts .

The learning curve to living with HIV can seem steep at first but you will get there if you do the work . You really are going to be OK and now is the time to start believing it .

There is no hurry so I would suggest that you work on accepting your diagnosis before you deal with how other people may react . I'm speaking about general social situations .

There are people you should always disclose to and that's doctors and dentist and of course mental health care professionals .

Welcome to the forums. I was diagnosed 4 years ago next month. I was in the hospital with PCP pneumonia, my vl WAS 500,000 and I had eight T cells. To say I was at deaths door seems dramatic but I was obviously closer than I had ever been. The diagnoses did not really come as a surprise. I had avoided being tested because I did not think I could handle the news mentally.

I had a doctor from MD Anderson who was treating AIDS in NYC in the 80's tell me to basically not freak out that this is not the same disease from 30 years ago. He told me that they could effectively reverse my numbers and life expectancy was normal. Within 4 months, my vl was undetectable and CD-4's had rebounded above 300.

I was scared but I held on to what they told me. Went to every doctors appointment. Get my labs done on a regular basis. I don't miss a dose of my med (Atripla). 4 years into this, my biggest challenge is maintaining eligibility for the programs that help with meds and doctors visits. Most days I don't even think about AIDS. Life can return to a "new normal" but it takes time. Everyone processes this stuff differently and in different ways but the reality is that the virus is treatable. Numbers are reversible. Life really does go on.

It helps to get some counseling if it is available.

PCP Pneumonia . CD4 8 . VL 500,000

TRIUMEQ - VALTREX -  FLUOXETINE - FENOFIBRATE - PRAVASTATIN - CIALIS


Numbers consistent since 12/2010 - VL has remained undetectable and CD4 is anywhere from 275-325

i took last week drug resistance test but i don t know what am i waiting for. i already started with meds cause my doctor told me to start cause he see i am anxious and willing to start taking them asap.
i don t know how bad can be result of that test?
is it possible that i have type of the virus that none medications can help me?

I would await the HIV genotyping results (aka, resistance testing) and continue your Truvada/Prezista/Norvir regimen for now.

Very unlikely that you are resistant to your current regimen. Protease inhibitors (PI's) like Prezista have the highest barrier to resistance. That means it's very hard for HIV to develop resistance to it, but it can happen.

Missing doses too frequently will do that.

Best case scenario is that your strain is wild type. That means any antiretroviral treatment (ART) regimen will work. In that case, your doctor will talk with you about simplifying your meds.

If you get to that point, trust me. It's mentally boosting to cut down on the number and size of the pills. The Prezista/Norvir tablets are huge!

Worse case scenario of multi-class resistance: Then your doctor will work with you on salvage therapy. S/he may keep you on your current combo and add other classes to help. I find this case extremely unlikely, though.

P.S. I hope you didn't get the Norvir capsule. Those need to be refrigerated, or they go bad after a month. Made my doctor switch me to the tablet after the first couple months.

Muratti,

It would be highly unlikely that you would be resistant to ALL antivirals, especially with you being newly infected.  I understand you being new to being poz and having so many questions and I would urge you to just slow down a bit and take it slow.  You are being effectively treated and even if your resistance test showed a resistance, it would not mean you would have no options for treatment.  You are also worrying about something that you have no control over at all.

As I said, I understand your fears, but you are not going to get sick and die in the near future and with the meds we have today, there is no reason that you will not live a very long time.  What I suggest is you just allow yourself to feel, whatever it is you feel, without needing to do anything about any of it.  You are still adjusting, so take it slow and be kind to yourself and that includes your body, mind and spirit.

Things will get better, I promise and the best thing for you is to reduce your stress and stop worrying about things that may never happen.

I did not see this response before I posted and you are not being annoying, simply by asking questions.  You mention that this is your first forum and we have guidelines to make posting as simple as possible.  I never want you to feel that you cannot ask questions, scream, cry or simply vent.  Annoying would be you telling us your tie does not match your suit.  Asking real questions about you and your life and sharing with us, who you are, is why we are here.

When I ask you keep all your comments in one thread, it is so we can see what is happening as we talk about your issues.  If you started different threads, it would be very difficult for you to get the responses you seek.

You have not done anything wrong and if you need some help with posting guidelines, just ask.

Yes, multi-drug resistant HIV strains do exist. However, they occur in people who are not compliant with meds or have participated in numerous drug trials with suboptimal therapies. Or you received transmission from one of these people. The key here is a virus with previous exposure to ART drugs. If the person who gave it to you didn't know about their diagnosis, chances are they weren't on such drugs.

If you are in none of these situations, it is very unlikely for you to have a multi-drug resistant strain.

You can waste energy worrying about something you don't know and have no control over. Or you can wait for the genotyping result. Then, go from there. You will know shortly and CD4's are okay and you're already on medications, so why worry now?

Hello Muratti,

I am posting to say Welcome and to underline the point that you have a good number off CD4 , an excellent place to start from. You also said that you feel "physically OK " or words like that. Keep reminding your self off that if you again start building up a whole lot off what if questions . You have done all there is to be done for now, time to work as hard on relaxing your self as well .

i know i should and i am trying. i am trying to see life beautiful as it was 2 months ago but it s just hard. there is that thick fog of thoughts, questions that overshadow my mind. especially when i am alone.

Most of us have felt the same as you but with a little work and time it will get better .

I barely even pay attention to my lab work or numbers anymore after 30 years of being poz . I had a few viral load blips pop up from time to time and my doctors were trying to go to great lengths to reassure me so I finally told them that I had seen it all in my time and was not the least bit phased by a few bad labs . You will probably feel the same some day .

What I am trying to say is you do not want to walk around living your life s a total panic obsessing over what if fears . Life is short for ALL human beings so do whatever it takes to get a grip on your fears . In the end I doubt people ever have final thoughts of I wish I had worried more but you may well wish that you didn't waste your healthy days wringing your hands worrying over what tomorrow may bring .

Keep talking about it and get some therapy if that is what it takes so you can make the best of today . If you didn't have a few bad days you wouldn't appreciate the good ones is the way I look at it .

Yes
Diagnosed in  May of 2010 with teh AIDS.

PCP Pneumonia . CD4 8 . VL 500,000

TRIUMEQ - VALTREX -  FLUOXETINE - FENOFIBRATE - PRAVASTATIN - CIALIS


Numbers consistent since 12/2010 - VL has remained undetectable and CD4 is anywhere from 275-325

No, he means tell us about yourself. You don't need to post a photo.

For example, you say that your English isn't very good (it's ok though), but you don't say what language you grew up with (what we English speakers call your "native tongue"). If you tell us, I might be able to give you some links to hiv information in that language. :)

A person with a good CD4 level and low viral load DOES NOT HAVE aids!

With your numbers you are unlikely to have much of anything hiv-related going on and you're not going to suddenly get sick and die.

You'll be just fine until you can start meds. You'll just have to trust me on that.

i did, thanks ann. but when i said my english is not good enough i didn't mean not good enough so i can't understand hiv or medical things. I do understand well. I don't get some basic phrases or idioms cause they are more slang...It s more that i don t feel comfortable writing english because my limited knowledge of it. My croatian language was my profession. When u r great in ur language it s very frustrating when u can' t express urself in another.

muratti, this is a pretty international multicultural group. you write in english better than i do. just get it out there, we'll figure it out. the only one you have to watch for is theyer, sometimes he just makes no sense at all

does anyone has like a 7 days diet, or schedule what to eat. i am bad when it comes to the food. usualy i would be lazy to cook just for myself and i never had particular time when i eat. i think that need to be changed.

Not sure about the punctuation - maybe thats just the younger, Internet style of no capital letters.  ;)

I dunno how deep you want to go on nutrition, but if you are ok to read something a bit academic and exhaustive, this book is used in introductory nutrition classes in universities:
