Hi:
I have to choose my medication next week and i donīt know what to choose. My doctor say that i can choose any pill. I worry about the lipodystrophy and the long term side effects.
Whay do you think is better? Complera, Triumeq or Dolutegravir+Truvada.
Thanks for your help.

The link below provides the thoughts on Complera, as well as Triumeq, from Dr. Gallant.  If you don't know, he is a prominent HIV doctor.  You will see he doesn't prescribe Complera much these days, due to it being taken off the preferred regimen list.  It has the calorie requirement.  He says in those who've developed resistance in his practice, it is either due to not being adherent or taking on a empty stomach.  But, there have been several here, who do great on it. 

You will also see his response about Triumeq.  He avoids it in people with cardiac risks.  It is still unclear whether the abacavir in it raises the risk for cardiac problems.  But, this seems to be an issue for those with existing cardiac risk factors.  You would have to get the blood test for Triumeq, to make sure you can take it.  Most pass that test and don't have the hypersensitivity issue.  Until the reformulated version of Tenofovir in Truvada comes to market, it should be avoided in those with kidney damage or bone issues. 

From everything I read, none of these meds cause lipodostrophy.

Both are great regimens. Its up to you to choose. Complera needs to be taken with a full meal every time. At least 400 kcal with some fat at least. Triumeq doesn't have this restriction but you need to be tested for abacavir allergy and there might be a potential for heart complications (vs kidney and bone loss complications with tenofovir - but a less harmful form of tenofovir is expected to be approved next year). Riplivirine also has neurpathy side effects, but not as bad as efavirenz (found in Atripla). Dolutegravir(component of Triumeq)  also have less long term studies done with it but have a more potent effect on viral load initially.


It really depends on your health and the state of your body function. If you have none, Triumeq might be good for you with medical supervision. We are all lucky to have so many options.
