Am i at risk? (I'd really appreciate moderators' comments)
« on: January 07, 2011, 07:45:47 am »
I am a 20 year old guy. I used to have gynecomastia (enlarged male breasts/moobs) until i had it removed by a plastic surgeon slightly more than a month ago (Dec.3).

Yesterday, i visited a sex worker and she did gave me a handjob and blowjob (with condom on). Prior to the handjob and blowjob, she sucked and licked my nipples.

I'm worried that i might get hiv since i have stitch marks around the lower half of my areolas (the surgeon made an incision there in order to remove the breast tissue). The stitches are no longer visible as they have dissolved 3 weeks ago but there are cut like patterns (no blood flowing out of them). In between the cut-like patterns there are needle-sized holes (due to the stitching) but no blood is flowing from the holes.

THumb, you are worrying needlessly. Nothing you have reported in anyway would have put you at risk for HIV transmission. Even if you hadn't used a condom to receive a blowjob it still wouldn't have been a risk.

As for where the stitch marks are, again that was no risk.

For future reference, the only confirmed means for the sexual transmission of HIV are unprotected vaginal and anal intercourse.

You're fine and there is no need for testing nor for further concern on your part in relation to HIV and this incident.

Thanks a lot Andy. Why is it that some sources (other forums and articles) claim that hiv can be spread via oral contact. Also, is it true that if i had a deep cut on my leg and if a hiv positive person's saliva came into contact with it, the chances of me getting aids is 0%?

The level of HIV in saliva is very, very low and not a means of transmission.

We're not responsible for what other sites and sources may say. We're very careful about what we put out here and it is based both on solid HIV science and decades of experience in the epidemic. Ufonrtunately both on the web and in print inaccurate misinformation is available that plays into people's worst fears without any solid HIV science basis.

Thank you Andy. I really appreciate your time answering my questions. I read an old post here about someone who claimed to have gotten hiv via a blowjob (without condom). Is there even a remote possibility of this happening?

It has never been documented in science, statistics, nor long-term studies conducted on three separate occasions on two continents. Receiving a blowjob is NOT considered a risk for HIV infection, any more than kissing.

Thumb,

I removed your photo from your first post. We do not encourage people to post photos of bits of their bodies as that might lead to the mistaken impression that we can diagnose anything here. We can't. I realise you weren't looking for a diagnosis, but your photo was inappropriate for the purpose of this forum.

As you have been told, nothing you are worried about was a risk for hiv infection. Here's what you need to know in order to remain hiv negative:

You need to be using condoms for anal or vaginal intercourse, every time, no exceptions until such time as you are in a securely monogamous relationship where you have both tested for ALL sexually transmitted infections together.

To agree to have unprotected intercourse is to consent to the possibility of being infected with an STI. Sex without a condom lasts only a matter of minutes, but hiv is forever.

Have a look through the condom and lube links in my signature line so you can use condoms with confidence.

ALTHOUGH YOU DO NOT NEED TO TEST OVER THIS SPECIFIC INCIDENT, anyone who is sexually active should be having a full sexual health care check-up, including but not limited to hiv testing, at least once a year and more often if unprotected intercourse occurs.

If you aren't already having regular, routine check-ups, now is the time to start. As long as you make sure condoms are being used for intercourse, you can fully expect your routine hiv tests to return with negative results.

Don't forget to always get checked for all the other sexually transmitted infections as well, because they are MUCH easier to transmit than hiv.

Use condoms for anal or vaginal intercourse, correctly and consistently, and you will avoid hiv infection. It really is that simple!

Some STDs are far more easy to transmit than HIV. Chlamydia, gonorrhea, HPV (the virus that can cause genital warts), herpes, all of these can be transferred through skin contact. I always recommend that any sexually active person get a full STD screening at least twice a year.

Unlike HIV, these pathogens do not require specific cells in order to infect. Also, they tend to be much hardier than HIV.

Here are the studies I mention. I apologize if there is overlap:

Human immunodeficiency virus (HIV) and many other viruses can be isolated in blood and body fluids, including saliva, and can be transmitted by genital-genital and especially anal-genital sexual activity. The risk of transmission of HIV via oral sexual practices is very low. Unlike other mucosal areas of the body, the oral cavity appears to be an extremely uncommon transmission route for HIV. We present a review of available evidence on the oral-genital transmission of HIV and analyse the factors that act to protect oral tissues from infection, thereby reducing the risk of HIV transmission by oral sex. Among these factors we highlight the levels of HIV RNA in saliva, presence of fewer CD4+ target cells, presence of IgA antibodies in saliva, presence of other infections in the oral cavity and the endogenous salivary antiviral factors lysozyme, defensins, thrombospondin and secretory leucocyte protease inhibitor (SLPI). The Romero study used male/female relationships almost exclusively, with either male or female partner positive. The viral loads varied widely, as did the treatment options (or lack thereof) during the ten years of the study.

The abstract of the study is also here:

Other relevant studies, many of which reference either/both Romero and/or Page-Shafer, among others:

Note the methodology for the Page-Shafer and Romero studies, and how they differ significantly from earlier studies which relied on post-infection patient report. In my opinion, that methodology dramatically increases the validity of the report.

Thumb,,

If you are sexually active, you should get tested at least once a year even if you use condoms. While condoms are very effective in preventing hiv infection, it is possible to become infected with some of the other, more easily transmitted infections even when using condoms. However, you are much, much less likely to get anything when using condoms.

However, this is an hiv website, not a general STI website. You did not have a risk for hiv.

You're welcome.

I would encourage you to not indulge in a lot of web searching and other like activities about HIV.

Keep it simple. Use condoms for intercourse. You were not at risk in your recent incident. If we thought there was even a slight risk we would say so.

As Ann suggested, anyone who is sexually active ought to have a full STD panel done at least annually.

Cheers.

AND...keep it simple.

According to Rick Sowadsky of the Nevada State Health Division Aids Program,

'I would also like to add that as time progresses, I'm finding more cases of persons stating that giving oral sex (exposure to semen etc.) was their only risk factor for HIV. There are also reports in the medical literature of people becoming infected specifically through giving oral sex. The risks for acquiring HIV by giving oral sex is less than that of intercourse, but the risk is real, and people have become infected this way."

Can i trust what he says?

No you can not. He like other researchers who do not use the scientific method, is using patient report as his primary source. This has been shown to be wrong in more than one study. Patient report, especially post-diagnosis, is not at all reliable. This person should know this.

However, you do not need to search more than a day in the past on these forums to find the sources we use for our assessments. I hope that you do so.

Thanks alot jkinatl2. Am i right to say that patient reports are unreliable simply due to a patient not revealing other risk factors such as unprotected anal and vaginal sex?

I guess i'll move on with my life. By the way, i really appreciate what you guys are doing. I hope that your website will eventually be the most visited aids/hiv-related site because you guys seem to be more knowledgeable than other sources. I wish aidsmeds/poz even greater success in its efforts to educate the world on issue pertaining to hiv/aids.

Thumb,

Yes, patient report is unreliable because people are often too embarrassed to admit to unprotected intercourse, especially anal intercourse.

And patient report is also unreliable because people don't always accurately remember what they did while under the influence of drink and/or drugs.

YOU did NOT have a risk for hiv infection. Give it up and get on with your life.

Thanks for offering to make the site to others.

However I would suggest your #1  resolution for the New Year in relation to HIV/AIDS be to use a condom everytime for vaginal and anal intercourse. If people will do that we will be able to lower the infection rate and lowering the infection rate is how to stop the epidemic.

Best to you for the New Year.

Hi, I came across this study on the World Health Organisation website. According to the study, the "consistent use of condoms results in 80% reduction in HIV incidence".

Are there any possible flaws in this study? Sorry for posting here.

Thumb, this is not intended to be used as a discussion site for whatever you may be finding on the web. Our specific purpose here is to discuss specific sexual situations in terms of level of risk.

In a general way I will say that consistent universal use of condoms for intercourse is essential to ending the spread of the epidemic. To that end we encourage consistent use of condoms for vaginal and anal intercourse.

Thumb,

There have been three long-term studies of couples where one is positive and one is negative. In the couples who used condoms for anal or vaginal intercourse, but no barrier for oral activities, not one of the negative partners became infected with hiv. Not one.

Use condoms for anal or vaginal intercourse, correctly and consistently, and you will avoid hiv infection. It really is that simple! Read the condom and lube links in my signature line so you can use them with confidence.

What should I do?

Today, I had vaginal intercourse with a prostitute using a condom. Once we were done, she withdrew the condom away from my penis. I noticed that while she withdrew the condom, some of her vaginal secretion on the exposed side of the condom came into contact with the head of my penis.

Secondly, I had a small fresh cut on my leg. I'm pretty sure that her vagina (and possibly some vaginal secretion) came into contact with the fresh cut on my leg.

Lastly, she licked my scrotum while giving me a blowjob with a condom on. I'm aware that HIV is not transmitted through saliva but I'm worried that she might have had bleeding gums.

Do any of the above acts place me at risk of HIV infection?

Appreciate your replies

Thumb,

Yet another no risk situation. Re-read your entire thread.

If you read the Welcome Thread before posting like you're supposed to, you will have read the following posting guideline:

Quote

Anyone who continues to post excessively, questioning a conclusive negative result or no-risk situation, will be subject to a four week Time Out (a temporary ban from the Forums). If you continue to post excessively after one Time Out, you may be given a second Time Out which will last eight weeks. There is no third Time Out - it is a permanent ban. The purpose of a Time Out is to encourage you to seek the face-to-face help we cannot provide on this forum.

Please consider yourself warned!

Use condoms for anal or vaginal intercourse, correctly and consistently, and you will avoid hiv infection. IT REALLY IS THAT SIMPLE!!!

HIV is certainly character-building. It's made me see all of the shallow things we cling to, like ego and vanity. Of course, I'd rather have a few more T-cells and a little less character. Randy Shilts
