lab results (reyataz+T+N)

Hey guys!!!

I just got back my first lab results since i had started therapy (reyataz+truvada+norvir). So, after 3 months on the therapy my values are:
CD4: 373 (25%)
VL: 459

Just some facts: got infected in January 2011, tested + in February, started the therapy on 16.12.2011. Values prior to the therapy: CD4: 284, VL: 120 000

I am doing really well on this combo, means i haven't experienced any (major) side effects, so i dont see any reason to change the regimen. BUT my doc thinks that values of BILIRUBIN are on the extreme top end of still acceptable numbers for someone on reyataz (bil total 75, bil dir 20) that's why he wants me to come back after a month to take another sample of my blood and to retest it. I really want to emphasize how well i have been dealing with this combo and how little problems i have had regarding its side effects  (apart from some yellowing of the white of the eye nothing ...and even this jaundice was very light and not noticeable ).

Any comments on my labs? Cd4 cells haven't really risen as much as i had expected, but VL has dropped significantly .....what about bilirubin? Are my numbers over the average for someone on the treatment with reyataz?

Hey Sevillano,

Congratulations on your numbers - looks like it's working out well for you, especially as you're not having any significant side effects. I'm quite surprised they waited three months from initiating treatment to do your bloods, but there you go.

I'm not qualified to comment on your bilirubin levels, but I am aware it can be a problem with reyataz, as it was something that me and my consultant discussed when I started treatment last month. For various reasons, we went with prezista + truvada + norvir. I gather prezista is less likely to cause hyperbilirubinism than reyataz, so it might be worth discussing prezista as an alternative if you feel it's a problem. It's a combination that's worked very well for me - bit of a dodgy tummy for the first three weeks  but everything's settled down now.

Sevillano, your CD4% has gone up quite a bit, so don't be too disappointed in your absolute CD4 result. You're doing well. You'll probably be undetectable on your next set of labs, so be happy!

A lot of people have an elevated bilirubin level in the first weeks/months of being on Reyataz and quite often this corrects itself after a while. For some people, it continues to be high. As Benji mentioned, there are other meds you can use, so don't worry about it too much.

Yes, but you started out at 19% and your overall trend is steadily going up. You need to watch the trends more so than concentrating on one individual result. You'll get used to it after a while. :)

Agree with your doc on bilirubin level topping acceptable range, but it might settle. Prezista (darunavir) is an easy switch. Keeping Reyataz but using less/no Norvir (ritonavir) is not as easy but possible. << the high bilirubin suggests you have more Reyataz than you need, there is a test to measure this

Matt ;) i have been waiting for you and your opinion ;))) hehe
We gonna wait till my next blood labs are done (11.5)...if values of bilirubin stay around  the same range, i am gonna suggest to wait another 3month period..as far as i know, high values (as long as they are not high as 25x more than normal) of bilirubin don't have any direct or inderect effect on any body part, so there is no reason not to wait a bit more...hmmm?
