Condom Burst with CSW, circumcised penis, PEP - Help

Hi,
On october 14th i was at a massage parlour and what was to be only a massage turn out to be also sex.

The girl was young (18 or 19) and new to this business but very cautious about sex because even oral sex was made with a condom. The main problem was that after the penetration when i removed my penis off her the condom was burst at the top of the glande. She was very nervous and started crying and the others girls there went to calm her down saying that this always happens and it looked like it was her first condom burst.

It was about 5 minutes of insertive vaginal sex and i really dont know how much time passed before the condom had bursted but i switched position one time and i thought the condom was ok but im not sure. After the ejaculation and i kept going inside her for a few seconds and neither i or her felt it was burst and only found out after i removed my penis and the burst was only on top where the semen is stored. So in the worst case scenario it was 5 minutes and in the best was only a few seconds.

After this i was also in panic. She showed me that she was a blood donor and was tested 3 months ago (Which doesnt mean much since there is the window period) but i was too worried since it was my first condom burst.

I am a 26 year old healthy male with circumcised penis. After the sex i checked my penis and was ok with no bruises or anything.

The next day i went to see a doctor and start a PEP treatment. He told me it was very hard to get HIV in a male-to-female vaginal sex and even tough she was a CSW (According to the governement the hiv rate in CSW in my country is 4.9%) the risk was in the low side of the scale but since she was a CSW i would start a 3 drugs PEP regime for 28 days. He told me since i was the insertive partner, it was a condom burst, my penis was circumcised and i was starting PEP my chances were good of not being infected.

I also went another doctor to ask for a second opinion and he went far as to say i should not be worried and dont even get tested. Wich was a very irresponsible position.

I finished all 28 days of PEP treatment without losing a single dose but one time i missed the time with a one hour and 40 minutes delay and other than that just 15-20 minutes give it or take between doses. The side-effects were hard, especially the nauseas.

I had no fever, swollen nodes or skin rashs. But since im very worried i wen to a dermatologist to check a few spots on my neck and he told me it was somenthing that i have for years and it is related to the weather and very commom. And also a had a little bit of sore throat and cold for a few days but then again almost half people i know was with the same condition probably because of the weather, so im not worried about this.

Sorry for the big text and any english mistakes (is not my language) but it is important for me to explain everything. I have anxiety problems and im also hypochondriac and this is killing me. I am felling worse every day with guilt. Is bad for me because i used condom (never did without) and it was only my 5th time with a CSW and i end in this situation.

I talked with some friends and  five of them told me that also had condom bursts with CSW and did not get infected.

I am just waiting 6 weeks after the PEP to do my tests.

I just want you guys to see if my risk of getting infected is high or low according to my case (circumcised, condom burst, insertive vaginal sex for no more than 5 minutes and maybe even very less, and PEP treatment).

I've yet to see the insertive partner end up positive following a condom break - PEP or no PEP - and I do not expect you to be the first. PEP isn't actually recommended in your situation, but testing IS recommended.

As you already know, you need to test at six weeks past your last dose of PEP. You'll have to confirm your negative result - and I have NO DOUBT that you will test negative - at the three month point.

You're just going to have to relax and get busy with other things while you wait to test.

Here's what you need to know in order to avoid hiv infection:

You need to be using condoms for anal or vaginal intercourse, every time, no exceptions until such time as you are in a securely monogamous relationship where you have both tested for ALL sexually transmitted infections together.

To agree to have unprotected intercourse is to consent to the possibility of being infected with an STI. Sex without a condom lasts only a matter of minutes, but hiv is forever.

Have a look through the condom and lube links in my signature line so you can use condoms with confidence. A correctly used condom rarely breaks.

Anyone who is sexually active should be having a full sexual health care check-up, including but not limited to hiv testing, at least once a year and more often if unprotected intercourse occurs.

If you aren't already having regular, routine check-ups, now is the time to start. As long as you make sure condoms are being used for intercourse, you can fully expect your routine hiv tests to return with negative results.

Don't forget to always get checked for all the other sexually transmitted infections as well, because they are MUCH easier to transmit than hiv. Some of the other STIs can be present with no obvious symptoms, so the only way to know for sure is to test.

Use condoms for anal or vaginal intercourse, correctly and consistently, and you will avoid hiv infection. It really is that simple!

[Very Concerned]

I am just a few days from getting tested but something is now disturbing me. About 40 days after finishing my pep treatment i think a pimple or other thing started growing in my upper cheek and three or fours days later it became like a nodule with a purple color and after five days without getting better i went to see a dermatologist. The doctor saw it and thought it was a inflamed pimple because of touching and gave me a cream to pass and told me to get in 10 or 15 days if it did not heal but i did not told him about my condom burst exposure or my suspicion of kaposi sarcoma.

My worry is: Is possible to develop kaposi sarcoma just 2 or 3 months after the exposure  described exactly above? My health is fine other than this but since i am very hypochondriac and anxious im not felling good now.

Anyway, right now im very bad about this and just hope that PEP worked and to come out of this ok.

Next post it will be my test result.

You do not have Kaposi sarcoma and you should fully expect a negative result . You had a low level risk and did a course of PEP so you need to relax and test , it will be negative .

It took me some time but last month i got tested for all std....and everything, hiv included, came back negative.

It was probably the worst day of my life when the condom broke.

I have to say here in my Brazil despite all the problems the HIV program is great. I started PEP right way and i believe this made the difference and it was all free. My company pays for a expensive health care provider but the hiv program is so much faster that i went to a public hospital. Kudos for that.

And i also have to thank this website for being the only space that i could share my fears.  It really helped me to hold on in a tough moment. You guys have no idea the much you did for me.
