Title: The transfer from medicaid to Medicare when disabled

Hello all !

I haven't posted here in quite some time. 

I do however, lurk, and read posts regularly.

However, I was hoping that someone could help me out with understanding the procedures with going from Medicaid to Medicare with regards to Part A, B and D. 

We already know that because of Sam's limited lincome from Disability and SSI, his Medicare part D premiums will be paid for by Medicaid "(I think)"...

However, the part B premium (Doctors) part is not really clear. 

I am not sure if forms need to be filled out about this.  I have called his case worker and left three messages over the last three weeks about this and have not received a return call.  In addition, I have searched the Medicare website and cannot determine if this will be an automatic change or that we will have to start paying these premiums. 

So anyway,

I am asking for your experienced answers.

There are generally state-specific publications regarding this and who to contact but the federal guidelines are similar.
