Broken condom - advice needed
« on: April 01, 2012, 12:32:42 pm »
Hi all,

I don't want to spout mumbo jumbo so I'll cut to the facts.

Friday 1am I had sex with a Caribbean CSW and the condom split exposing the tip of my penis. I didn't ejaculate and we noticed the split immediately (<10s) and so stopped the session.

I asked if she was clear of any STIs and she said yes and that they got tested every 3 months. I obviously have to take this with a pinch of salt.

Saturday 1pm I went to hospital to get some blood taken and explained my situation to the doctor. I had read about PEP and asked if this was an option, being that the possible exposure was within 12 hours. After some talking I got given a 3 day pack - however we were agreed that this wasn't necessarily a high risk exposure - I weighed up the risk and decided I'd rather do 28 days of nausea etc rather than hedge my bets and still have to be tested 3 months down the line.

So as of now I have been feeling the side effects of the PEPs and it's not great but not unbearable by any means.

I would like some advice from someone who has knowledge and experience on matters such as these.

Do you think it warrants me to go on PEPs for 28days? What is, if any, the other options available? I plan on going to my clinic tomorrow morning to get the rest of the treatment in absence of a solid answer because Im unsure of everything right now :(

Am I right in thinking that's have to wait 12 weeks as of the date which I end the PEPs to have the first proper blood test outside the window period?

Any help is greatly appreciated.

Thanks

Newbie,

Yes, if you continue with the PEP it will increase your window period by four weeks (the length of time PEP is taken).

You don't really need PEP - I would expect you to test negative with or without it - but it's your call.

Hiv is significantly more difficult to transmit from a woman to a man.

PEP isn't recommended for the insertive partner following a condom break, particularly when the receptive partner's hiv status is unknown. It's not NO risk, but it certainly isn't high risk either. This very brief incident of yours is very much on the low end of the scale.

I've yet to see the insertive partner end up hiv positive following a condom break and I do not expect you to be the first, PEP or no PEP. Again, whether you continue or not is your call. I would advise against it.

Thanks for the advice Ann.

I went onto the full course of PEP anyway - mainly for my peace of mind.

I got a pre-lim blood test done on Monday but results aren't back in yet. Getting a full STD check in another week and then all the relevant blood work done 6 weeks after end of PEP and then 7 weeks after that.

If anyone is interested, the side effects haven't been too bad for me TBH but will keep updated in this thread 9 and 15 weeks from now.

Thanks again for you're advice though - it's nice to know there are people willing to give the time to explain these things whenever we need it most!

Hi all.

Just an update to let you know that all my results came back negative.

Thank you so much for the advice and words over 3 months ago, they were invaluable at the time and I know now how they can help calm fears. Ann, I hope you realise just how much you help people when they come onto these forums and I can't praise you enough.

So again, thanks from the bottom of my heart.

Glad to hear about your negative test result. Just want to be sure that it was taken at 3 months after the completion of PEP in order to be conclusive.

Hey Andy. It was 14 weeks after exposure so that means 10 after PEP. I had said to the doctor I would come back in a few weeks for another test but she said it was pretty conclusive after 10 weeks.

Would you agree, or is the extra 2 weeks very important?

I wouldn't expect the result to change. You decide if you're comfortable with this happy result.

I feel so stupid now for not leaving it another two weeks! It was on the NHS so I'm not sure if the conclusive result period changes depending on what test is used?

Are there large odds that the outcome will change or are they relatively insignificant? Any help would be greatly appreciated here Andy.

It would be very, very unusual for your negative result to turn positive at this point.

So re-testing is something that I expect would only give you another negative result.  Really.

Newbie,

You never actually needed PEP in the first place and your ten week post-PEP result is not going to change. I've yet to see the insertive partner end up positive following a condom break - PEP or no PEP - and you were not going to be the first.

You do not have hiv and any further testing would be for your peace of mind ONLY.

I suggest you do not waste further NHS resources and just be happy with being hiv negative.
