Im currently in indonesia where the hiv rate among SW is 1 in 4 and 24 hours ago i foolishly made a major drunk mistake. I didnt wear a condom for anal and finished with a bj with a SW with an unknown std status and i cannot find again. I am uncut. How would you rate my exposure? Should i get on PEP as soon as possible? Thank you very much for your time.

Were you the insertive partner in the unprotected anal? The unprotected anal was your only risk.
If you were the insertive partner, your chances of testing negative are very good as it was a one-off and hiv is much more difficult to transmit from the receptive partner (bottom) to the insertive partner (top).
If you were the insertive partner, PEP is your call, although it probably wouldn't be a bad idea.
If you were the receptive partner, then you should get PEP as soon as possible.

Insertive. How bad are the side effects on PEP? I really dont know what to do. The insertive anal was for a few minutes but no cum untill about 5 minutes of bj. I dont want hiv is all i know. Also, i wont be able to access PEP til the 35 hour mark. Is this much less effective for the PEP?

The meds used in PEP will effect each person differently. Some people experience no side-effects at all, while others have a rough time.
The standard PEP combo is Kaletra plus Truvada. However, Truvada on its own would be fine for your situation (it's two drugs in one pill) as you were the insertive partner and at a much lower risk for infection. You should not experience any major side-effects on Truvada, other than perhaps an increase of gas (farting). Truvada on its own is an approved PEP combo.
The two drugs that make up Truvada are tenofovir and emtricitabine. You may have to get them separately in Indonesia, I'm not sure. You can read about the various hiv meds in our Drug Information (http://www.aidsmeds.com/list.shtml) pages.

I accidentally re-opened this thread again and ended up re-reading it.
I noticed you edited your last post while I was typing my response to you, and I ended up not seeing the last line of your post:

PEP needs to be started within a 72 hour window period in order for it to be effective.
The earlier it is started, the better, but 35 hours is absolutely fine.
One thing - make sure you have a rapid test done first before you're put on PEP.
Pre-PEP rapid hiv antibody testing is standard protocol (that occasionally gets overlooked) to make sure a person is hiv negative before PEP is prescribed.
Good luck. I really don't expect you to test positive over this either way, but PEP will pretty much guarantee a negative result.
