Scared
« on: October 09, 2013, 12:32:59 pm »
I made a massive mistake of having unprotected sex with CSW (once anal and once vaginal). I am uncircumcised and I was the insertive partner. I do not know her HIV status. I have the following questions:

After how many days of the possible exposure should I get the test done?
Should I start PEP treatment? Within how many hours of exposure is it effective? Is it not effective at all after 72 hours?
How high is my risk of being infected - very high I assume.
Are there any symptoms which may indicate that I have been infected?

Many thanks.

You did have a risk and you will need to be tested for HIV . You can test 6 weeks past any possible exposure and again at 3 months to confirm the results .

Pep is highly effective if imitated within 72 hours of exposure . You go go to the emergency room or a clinic and discuss you options with PEP .

Anyone who is sexually active should be having a full sexual health care check-up, including but not limited to hiv testing, at least once a year and more often if unprotected intercourse occurs.

If you aren't already having regular, routine check-ups, now is the time to start. As long as you make sure condoms are being used for intercourse, you can fully expect your routine hiv tests to return with negative results.

Don't forget to always get checked for all the other sexually transmitted infections as well, because they are MUCH easier to transmit than hiv. Some of the other STIs can be present with no obvious symptoms, so the only way to know for sure is to test.

Use condoms for anal or vaginal intercourse, correctly and consistently, and you will avoid hiv infection. It really is that simple!

Many thanks for the reply. I realise it's a long shot, but would you know of any hospital where I can get PEP in Mumbai, India? Thanks again.

Thank you for this forum. It is extremely helpful. I got my test results today(tested 7 weeks post unprotected sex). I had myself tested for all STIs and the results are all negative including for HIV. Hopefully this won't change in the 12 week test. What are the chances it will change?

Since this is my last free post and other people may read it during their period of anxiety (like I read many posts by other people), there are a few words of advice I'd like to put here:

1.  Don't self diagnose. The stress will make your body exhibit every conceivable symptom. I felt I had got oral thrush, swollen lymphs and rashes.

2. Always use a condom. I can tell from personal experience that the risk of STIs is not worth any amount of pleasure or whatever. If you don't want to use a condom, get into a long term monogamous relationship.

3. Never mix alcohol and sex. You'll not be able to have proper sex and you'll take unwarranted risks.

4. If you feel you have had the risk of exposure, please initiate PEP immediately. It's difficult to get in many countries (especially developing countries), but you have a 72 hour window (i.e. 3 days), which is enough to figure out. Don't hesitate at all.

Um, no. Not good advice at all.

A lot of people "feel" they've had a risk when they've had nothing of the sort. Hiv meds are expensive and often cause side-effects - you should not be advocating that people go on PEP when they "feel" they've had a risk.

For example, your risk of insertive unprotected intercourse does not really warrant PEP and it is not recommended in the guidelines when the receptive partner is of unknown hiv status.

PEP should only be taken when there has been an actual risk. End of story.

Hopefully this won't change in the 12 week test. What are the chances it will change?

The vast majority of people who have actually been infected will seroconvert and test positive by six weeks, with the average time to seroconversion being only 22 days.

A six week (or more) negative is highly unlikely to change, but must be confirmed at the three month point.

I do not expect your seven week negative result to change when you confirm at the appropriate time.
