By the way, its good news.  People already suspected this and have used Truvada as such.
By the way, its a shit storm. 
Its going to prevent some transmissions - that is why its good.
But tease out some of the possible downsides.

For example, someone is HIV+ and doesn't know it. Takes their Truvada 4 days every other weekend for 2 or 3 months.  not nice.

Yeah, there is that risk.
Just Hope people check themselves first before taking it.
They should put it on the label.

But, Truvada (Tenofovir/FTC) stay in the blood not for long. Unlike nevirapine and Efavirens which stays for weeks.
So, I think resistance would be lesser to happen, since stop at once. Even if someone has HIV.

I am pro- PREP and I even like this new research finding.

However, I do often see the pros and cons and I do believe the argument of the "cautionists"  who see PREP as a permit to bareback, for some of the potential market at least.

If I were on the board of a company that makes HEP C treatment, I would love this PREP news - because people are going to use it to bareback. (People will be counselled to use it with condoms, but some will obviously ignore that.)

Since the HEP C cure costs so much, it will be a long time before HEP C is wiped out.  So there will be many more HEP C cases and, thinking like a capitalist, there will be that many more billions of profits on STD treatments.  nice

This is good news.  I agree with Mecch that Hep C, and resistance as well as failures due to people trying to time doses is a concern.

Does PEP really need to be for 30 days? Think of the cost and ability to offer it to more people if 2 weeks or even 1 is sufficient if you start right away...

I am in favour of PREP because we shouldn't penalise responsible people because other people are irresponsible.  that what I dont get about the anti-prep people.
