Scared to get tested/Acute HIV sypmtoms
« on: May 10, 2008, 08:30:17 pm »
First let me say how grateful I am to this site, I have been going through such agony over my situation and have found so many strong and insightful people on the site, that it makes me feel every thing will be alright.

A little about my situation, I was involved with a man about 2 months ago, and on one occasion March 21 the condom broke and he ejaculated inside me.  He stated to me he was HIV negative, which he now states he has never gotten an HIV test. I contracted Chlamydia shortly after this incident and have recently found out I am pregnant. I have read so my articles about acute HIV symptoms and this has me really nervous. I have had a fever 99.9-101f for 4 weeks, sore throat, night sweats and swollen lymph nodes, rash and yeast infections. Is this considered acute HIV symptoms?

The stress is causing problems in my pregnancy and the doctor suggests that I get tested to know for sure. My question is how accurate would the test be after 2 months and are there any suggestions on where to get tested. My doctors seem very cold about the situation, is it better to go through an AIDS clinic, the health department etc. Also, would you recommend a support person, friend family to come to the appointment. Any suggestions would be appreciated!

It's very important that you be tested for HIV. In most places HIV antibody testing is performed routinely on pregnant women.

The window period for HIV testing is three months, but if the stress is too much you can test now. A negative result at 8 weeks is usually an excellent predictor for a negative result at 13 weeks. If you do test now remember you will have to test again 3 months from the date of the unprotected sex.

You should also be aware that pregnancy can trigger false positive ELISA tests. You might want to speak to your doctor about specialised HIV testing to account for potential false positive results. If you're having difficulties with your doctor then a specialist HIV clinic may be a better option for you.

I should add that one unprotected sexual encounter is unlikely to see you infected even if the fellow in question is HIV positive. HIV is a fragile virus which is difficult to transmit. Nevertheless you must be tested.

The average time to seroconversion is 22 days. All but the very smallest number of those who are going to seroconvert will do so within 4-6 weeks after an exposure to the virus.

I agree with Matty the odds are well in your favor that you will test negative. Fingers crossed for you and keep us posted.

I have corrected my posting possible exposure was March 21st symptoms started around the middle or April, this may also be due to stress since this is aroung the time I found out I have Clamydia and was pregnant.

Thank You!

I did a quick test which came back inconclusive. What does that mean?

I also have strange symptoms that I cannot explain can the following be caused by HIV:

Headaches
Dizziness
Feet and hands keep falling asleep
Soft Stool/Diarrhea

How long does seroconversion last? I had a fever and sore throat for 4 weeks.

Inconclusive, which is a not unusual result, means you have to re-test. It means it didn't clearly show either positive or negative.

None of the symptoms you are reporting are in any way HIV specific. ARS lasts usually for a week or two. Because the symptoms resemble so many other illnesses they can never be relied upon to know about your HIV status.

I suggest you re-test. You can wait until 13 weeks for a conclusive result. If you test negative earlier than that you should re-test at 13 weeks. Your having chlamydia is irrelevant in terms of HIV being an issue.

Good luck with your re-test. Keep us posted.

Cheers,

Thanks Andy! I do have one more question if you don't mind. I am terrified to go back and get re-tested. My question is what are the pro's to finding out if I'm HIV+? I'm scared of knowing and living the rest of my life thinking I'm on the clock. It seems better to just live life blindly and hope for the best. I have become one of those people who swears I'm positive everything I read, see or hear has to do with HIV I swear the universe is trying to tell me something. I know this seems crazy but really why would anyone want to know except for not spreading it to others. Please help me maybe bring me back to earth. I have an appointment in 2 days for my retest and am debating if I should even go.

Still,

If you are positive, you need to know now so you can take the necessary steps to ensure that your baby is born hiv negative. If you do nothing, you greatly increase the chance of passing the virus on to your child.

However, you need to know that false positives are fairly common in pregnant women. Your inconclusive test might well actually be a negative test, but the only way you'll ever know is to go and get further testing done.

You owe it to your unborn child as well as yourself. There's worse things in life than being hiv positive, but you don't want to be responsible for infecting your child - when it can be prevented - do you?

Go get those tests done. Now.

Ok so I sucked it up and took another Rapid HIV test. The clinician stated it was "very negative" I know I need to be retested at 13 weeks but my question is how accurate is this test 8 weeks post exposure? Another silly question is what if there wasn't enough saliva to accurately test for antibodies. Does this happen?

Still,

The vast majority of people who have actually been infected will seroconvert and test positive by six weeks. Your eight week negative is very unlikely to change.

If not having enough saliva to test for antibodies was a problem, the oral tests would not be approved for use because a problem like that would render it too unreliable. The oral test IS approved and it IS reliable.

Now relax and get your confirmation at three months. I fully expect it to also be negative and so should you. Keep yourself busy in the meantime and the wait will go quicker.

Thank you Ann, I have a question though it just dawned on me that when I took my original test that came back "inconclusive" the test showed only one line in the C area and when I did the 2nd test it also showed one line in the C area, which the clinician stated "very negative". My question is how can that be they looked exactly the same. Could they have stated inconclusive since I tested so early? I contacted the clinic and they were unhelpful. I took the oraquick rapid advance test if that means anything.

I do have other questions if you don't mind.

Are false negatives high with the oraquick?
Can you seroconvert and fight off the HIV infection?
Do you often see a test go from negative at 8 weeks then positive at 13?
Would you recommend getting a PCR test?

The reason I am asking is that it just seems so strange to me that I have all these symptoms that were at a time when seroconverting should have taken place yet I tested negative. I'm not complaining, I'm relieved at the moment. I just can't figure out what is wrong with me physically all my blood work, x-ray's, cultures taken by my Dr. are negative they can't figure out what's wrong either. HIV seems the only thing that would make sense.

Thanks for all your help!

Still,

As 1) I'm not a mind-reader and 2) I wasn't there when you took the tests, I cannot possibly speculate on what they meant when they said what they said. You'll have to ask them. I understand that you have, but maybe you need to talk to the person who actually said these things to you. Personally, I don't see the point of pursuing this issue. Your last test was negative and that's what counts. Remember, pregnancies can sometimes cause false positives - and that goes for indeterminates as well.

An oraquick test taken at the three month point will not yield a false negative, except in VERY rare instances where the patient is on chemotherapy for cancer, anti-rejection drugs following organ transplant or has been injecting street drugs, several times a day, every day, for YEARS. Even these people will normally test positive within the three month window. I trust you don't fall into any of these categories.

Technically, if you've seroconverted, you've been infected. Seroconvert refers to the point at which there are enough antibodies in the blood to test positive. The blood (sero is a prefix which refers to serum/blood) changes (converts) from hiv negative to hiv positive.

I've never seen a six week negative end up being positive at three months, much less an eight week negative.

Again, I do not expect your result to change and neither should you.

So, I'm not sure where to post this. My curiosilty got the best of me and I got another Oraquick test done which not surprisingly came back positive. The clinician stated this test is not affected by my pregnancy, not sure if I should believe that. I took another test which should be back in 2 weeks.

I am concerned since I am pregnant and after 2 weeks will be into my 2nd trimester. I would like to get the advice from HIV+ pregnant women or HIV+ mother on the risks and benefits to having a child being HIV+. Would it be appropriate for me to post in the positive women forum?

The matter of your HIV status remains unclear so no, you should not be posting in any of the HIV+ Forums.

Do you have a doctor? That's who you should be talking with about this whole subject. You can get some information here: http://www.avert.org/pregnancy.htm as well by googling pregnancy and HIV on the net.

I'm still not persuaded that you have reliably tested positive so that is the most immediate issue to address. And I think you should be discussing this with your doctor.

Still,

A positive result on an ELISA rest must be confirmed with a positive Western Blot result. As you've been told, pregnancy often causes false positive result and we've had several women over the years posting in this forum who have had false postiives due to pregnancy. The clinician is wrong.

Did this person not give you follow-up information or Western Blot testing, seeing as how he seems to think your positive is genuine and he knows you're pregnant? Something isn't adding up here.

Until you've actually been confirmed hiv positive with both a positive ESLIA and a positive Western Blot, then this is the only forum you should be posting in.

I really hope you guys can clarify, I have gotten 2 positive Elisa results followed by 2 inconclusive Western Blots. I have taken a PCR test which should be back in 2-3 weeks. The infectious disease specialist at my hospital brought a few things up that has me very concerned. She stated that an inconclusive Western Blot probably was due to the test being administered too early ( i know you have told me differently). My main concern was that she stated pregnancy does something to the immune system so my body doesn't reject the fetus and could prolong getting an accurate HIV result to 6 months. Have you heard of pregnancy doing this? Do you think an 8.5 week negative result is still a good indicator that I have not been infected? Also, what is a PCR test? I know it tests for the virus itself but does it verify infection?

Being pregnant does not lengthen the converson timeline. Being pregnant does have a history of causing a false positives. Yes you tested to early. The only approved supplemental PCR test is the PCR-RNA which must be followed up with an antibody test.

Still,

Regardless of what your PCR test comes back with, you are conclusively hiv negative. ELISA trumps PCR.

I have read information on the false negatives with OraQuick Advance Oral swab test, which my clinic is no longer using due to the Seattle study.  I was tested negative at 8.5 and 13 weeks with this test via oral swab and thought I could put it behind me. I did get a positive PCR test but was reassured these tests are inaccurate.

Would you recommend being retested?

I would like to add it's been 7 months since exposure and have been unwell the whole time, I am also 7 months pregnant.

RapidRod,

I didn't come here just for kicks. Trust me with a 9 year old and another baby on the way I have better things to do. If you are so sure my previous tests were conclusive, please tell me my recent test was probably a false positive or my pregnancy affected it some how. If that is possible I would love to know. My last and only possible exposure was in March so I couldn't have been newly infected. Again, if there is anyway my tests could have been wrong let me know so I can bring it up to my dr.

Yes, I would bring it up with your doctor. This is way too important to leave any doubt about whether you are or aren't HIV positive. Like Ann I seriously doubt that you are, so don't go rushing to move over into the HIV positive section. Tell him/her you want to re-test or know why not to.

Still,

You do indeed need to speak to your doctor about this. Pregnancy is one of the conditions (along with some auto-immune diseases) that can cause FALSE positive results. This needs to be carefully investigated before you take it for granted that you're positive.

Good luck.

I apologize for all my questions but I don't seem to know where else to get straight forward information. I spoke to my doctor and asked for another test which she clearly stated was not needed as a confirmatory test was done. She was very rude in stating why I wouldn't trust the PCR and WB test and said I am avoiding the fact of my positive result, she said my 13 week test was probably a false negative.  I spoke briefly with the Infectious Disease Specialist who won't take my case since he has never dealth with someone pregnant and refered my to a larger hospital that can't see me for 2 weeks.

I don't know where to go, what test will accurately tell me my status. I can't afford to go anywhere private but am terrified it'll be too late to prevent transmission to the baby (if for some reason I am +).

Could the WB and PCR both be false positives?

We'll everyone I saw an infectious disease specialist who deals with HIV and pregnancy and can't say it was a very positive experience. She was concerned that I had an amnio and the possible exposure to the baby. I'm suppose to start AZT but want more info first.

Unfortunately, I did tell my partner about my situation who took a blood rapid test which came back positive, he took the confirmatory test which I don't think I'll ever know the results of since he took the rapid test as a definite "positive" and is now in jail for assault. That's my sob story, I did feel I could come to this site for support but didn't feel very welcomed in my last post.

Since the situation still remains unclear as to your status, I suggest you call any AIDS service (or women's support) organization in your area, explain the situation and ask for help in moving forward.

I honestly don't see there's anything more we can do for you here until there is reliably established status result one way or another.

If in fact you ultimately are confirmed as HIV+, great advances have been made in protecting the negative status of in utero babies and there's every good chance the baby can be HIV negative with proper meds for you during the pregnancy.

Good luck in getting this very troubling situation clarified.

I;m sorry if I come off rude, but I've already done that I've spoken to every agancy in my area and outside and all I keep hearing is "sorry mam your test confirm your HIV+" This is suppose to be one of the largest online HIV forums that offers advice and support, yet your stating my results are still unclear or why. What test am I missing PCR, Elisa and Western Blot all positive? What other test should I take?
