New mail order procedures?

Several months ago, I switched my regimen.  We always talk of building a reserve which I've done for ages.  I noticed today when filling my pill box that I only had enough new meds to get me through next week and a refill had not been scheduled.  I usually get a call from them when it's time to order.

So I called them to fill my script and I questioned her about why it's going down to the wire each month.  Apparently, they can no longer fill scripts in advance like before.  They must be filled no more than 1 week early and that's to allow for shipping.  My expected delivery is next Tuesday and I'll be out of my current meds on Wednesday.  All it'll take is one screw up to throw me into an "out of meds" predicament.

Is this a CVS issue or is everyone experiencing the same thing?  My dumb ass should have stayed on my old combo for a while to build a reserve, but it's done.  Oh well.

Greg, I thought you had the option of picking up your meds at a local CVS.

I do.  But we don't have a local one and I actually prefer them to be shipped.  Not sure if I can have them filled quicker in store but I might need to do that a couple of times if I can do it a little early to build up a reserve.

I would be willing to bet that you could pick them up at your nearest CVS 7 days in advance. Do it 2 or 3 times and you will have a good cushion.

I had this problem just recently when my insurance switched Atripla from its "specialty" program (which UPS'd a 90-day supply to me with plenty of time to spare) to its Tier 2 drug program (regular mail, taking up to w week).  CVS Caremark administrates the program.  Since the transition happened at the same time I needed a new supply, there was considerable confusion ("specialty" order canceled, but "regular mail order" not placed because insurance said "specialty" order was filled, etc.).  It took several calls to straighten it out, but I did straighten it out and got the meds two days before I ran out (CVS did fed-ex the order and did pay for the mailing since they did acknowledge the confusion, which was good of them to do).  Next time, I'll need to call a week or two earlier to make sure that the order is on the way.  I'd rather get the supply from the pharmacy, but it's far cheaper to get Atripla through the mail ($80 for a three month supply through the mail, instead of $1,500 if I got the same supply retail).

That is ODD.  I never had that occur before. They (I have CVS, too) usually won't let me order more than three weeks in advance, but they have never flat out turned me down with a week to go.  In fact, I just switched doctors and I had a prescription filled with two bottles left from my previous doc that was expiring, and then the new doc filled out another prescription that (I assumed) would take over at the end of this one. I got the shipment unannounced, so I'm two bottles ahead. 

I just called CVS to inquire because you had me worried.  What I found out was that with the three month prescription that should run out October 28th, it is available for re-order on September 20th.  I inquired further about your predicament and the person I was speaking to had no answer.

     Hi  Greg  :)  ,

                I may be playing with fire here ?

          Whenever my Doctors change my meds I stay on the old combo
      for a month or so  ,so as to have a back-up supply .

       This usually works out well . A few months ago I had to go back to
    Taking Reyetaz because my meds got lost in transit ( read as stolen
     from mail box )    , I was glad I had a back up drug . The V.A. is good
     for refilling on time  for the most part .
     Right now I am waiting on Citalapram , I will be out this week . Not sure
    where my pills went ?   
     It's not good having a mail box  a half mile from the house  ::)

Could it be something that your doctor is doing in the prescription process??  I'm not sure - just asking - if maybe he is putting some qualifier in the record "not before"?  I'm not entirely certain how this works with the interface between the doc and the insurance but I would guess that it is very convoluted and difficult. Maybe check with the doc/nurse to see what exactly he/she are telling them in the prescription process?

I too use CVS Caremark. Only have received one month from them this far. But should be receiving my next month soon, as I am down to about 14 pills. They told.me they would call me when it was time to fill again. I received my script on the 21st of July when should I expect to receive a call?

My own logic failed me (or lack thereof).  I previously was receiving a 3 month script that was changed a year ago to monthly.  They could fill a 3 month script 3 weeks early which meant in one year you could build about an 8 week reserve.

Being monthly, they fill it one week early but since I choose mail order, the shipping eats away at the time that would be used to build a reserve if I went to a brick and mortar pharmacy.  It didn't dawn on me previously because I already had a nice reserve.

I have regularly rec'd a call from them about a week beforehand with no problems.  They have even expidited the order a couple of times at no additional cost.  So it really is up to us to monitor our supply and not solely rely on them to contact us.  That's a great benefit but does not negate our individual responsibility.

I'm still content with receiving them from FedEx or UPS (can't recall which one delivers them).  I also have a fall back since my sister-in-law is a pharmacy tech at our small local pharmacy.  She can always get my meds in a couple of days if needed.

You should be receiving the call anytime now.  I'd check with them in a couple of days if they've not contacted you.

I believe this is because it isn't the pharmacy that dictates these things, rather it is the entity paying the pharmacy -- either the insurance company or the employer (for self-insured employers) - or perhaps a combo of insurance company and employer.

I, too, use CVS Caremark -- my Atripla is still Specialty (not Tier 2) and I get my 90 day refill call about 3 weeks early.  Well, except for the call I got yesterday to finalize my order which was just 3 weeks SINCE my last refill.  I called, they said, Oops and I'm not getting it that early.  ;)

I can't have mine FILLED at the local CVS, but I CAN have it shipped there, so that I don't have to be home when UPS arrives and/or worry about it being outside, stolen, etc.

You're correct.  After some checking, I discovered that I can pick it up at the brick and mortar store but not have it filled there.  Kinda doesn't make sense as they carry the meds in stock but it is what it is.  It's probably a dumb request, but I'm planning on discussing with my doctor if he can order 1 script of Isentress and alternate every other day with Norvir and Prezista in order to build a couple of weeks reserve.  I believe being UD would allow this option?

So i live in this apt building and the intercom is broken.I asked the local aids center,arcw,if i could pick meds up there,and yes i can and do.A little ticked cause they sent me the old kind of norvir that needs to be refrigerated this time.Without warning.Not sure how long it was without refrigeration.Didn't know they even made that kind anymore or why they do..yuck.And traveling would be a pain.
