Brief Insertive anal and on Pep!

Would you please provide with some information concerning a potential risk, condom broke, i did realized but I remain inside with out protection for like 2/3 more minutes until finished.... It was anal and I was the top with a unknown HIV status sex worker, no blood visible and it wasn't to rough...I know it was stupid (the heat of the moment), I've been in Pep for almost 3 weeks (Kaletra & 3tc= lamiduvina/zidouvina) after 6 hours of the potential exposure and not coping well with the medications.

Questions;
1-I use to take a raw garlic everyday before the incident, I was able to read a post that with Kaletra there is no real problems because Rotovir works a potential booster? Not sure if 3TC Lamiduvina (Epivir) and Zidouvina *Muvidina? Which I don't think still exist in the USA? Couldn't find any data on regards 3TC and Garlic, and that's a major major concern?
Going through Pep with a 50% efficacy and comprised treatment due to lower levels of the drugs in the blood? Read an article on the net that even after 10 days of wash down period of not eating Garlic there was a 35% reduce amount of medication on the blood? However it doesn't mentioned which type of medication was the study analyzing? Could any one please enlighten me with this if possible? As well I take strong cinnamon tea with Honey twice a day for sugar in the blood purposes and weight control and I stop after two weeks of Pep regimen? Heard that Cinnamon as well screw treatment? I guess I'm doing all wrong....! Bad F.. Karma!
2- I'm fully aware that only 6 to 12 weeks after the finishing of Pep I can start performing again HIV test, However the doctors down here suggested that after finishing with Pep is advisable to do a test and it very unlikely that it will change? Don't think so based on older post from Aids meds?
3-What can i take to avoid the stomach problems the drugs are very strong, the mentioned me lots of yoghurt? Is that correct?
4-I'm spoke nicely with the sex worker and she has no problem doing a test with me as long as I compensate for her time? However not sure how reliable could that be, and how long should we need to wait before that? worst case scenario if by some reason (hopefully not, but if she was infected one day prior to our incident does it apply the six weeks period as well as you always suggested for almost conclusive test? Does that apply with potential positive results? I read on your forums that the vast majority will seroconvert at 22 days? Could apply that principle?
I don't want to go into the what if.... bla bla.....but in a worst case scenario= Pep compromised due garlic and Cinnamon Tea, Sex  partner HIV + with high viral Load due to acute infection, what would be my odds of coming out well? By the way just for the record, she didn't seem to be concern about the incident plus she never ask me to used a condom on the first place? I assume that since she is some how very young and new on the business is very naive......which by all this I'm trying to create awareness for her as well.
My first baseline STD blood test came all well after one week of starting Pep? Is that reliable? HIV Doctors in here don't have the same guidelines as you guys

Pedro,

1. The garlic isn't going to make that big a difference. Calm down.

2. We recommend that the earliest one should test is at six weeks following a risk, or six weeks following the last dose of PEP. Or you could do what your doctor suggested. Any result won't be conclusive until three months post-risk or post-PEP.

3. Yogurt is good. Peppermint is good too. Have a look at this Lesson for more tips.

4. There's no point in making her test - you will never, ever know your own hiv status through testing someone else.

Most STIs can be tested for at around two weeks following a possible exposure. One week is a bit soon.

You are highly unlikely to end up positive as the insertive partner following a condom break. I've yet to see it happen in these forums, PEP or no PEP. You really don't need PEP.

Ann I appreciate your prompt reply, tks for the lessons link and reassurances, however I somehow have a conflict with the definition of condom breakage in my case, that's what I did classified it as brief anal, I was fully aware when the condom broke but unfortunately was unable to withdraw for 2/3 more minutes? I wish I could pull out immediately, but didn't happened, so even if I would like it to consider as a condom breakage it wasn't...! I didn't see blood, it wasn't rough either the sex act, plus I'm not the type that can presume of having a big thing.....and I'm circumcised as well? Sorry for being so explicit.
But does your assumptions apply as well for a brief insertive anal scenario as i did described?

Don't apologise for being explicit. We are sex-friendly here and we value honesty over being polite.

I fully understood that you kept going after the condom broke. I still would not expect you to end up positive following this brief incident.

Being circumcised is in your favour. There are only a very few, very specific types of cells that hiv can infect. On the penis, these cells are found in the urethra (where you pee/cum from) and on the lining of the foreskin (the bit you can't see when the penis is soft and the foreskin lies over the head). And that's it. When you don't have a foreskin, the only target area hiv has is the urethra.

I honestly think you're going to be ok.

PS... A correctly used condom rarely breaks, so make sure you're using them properly in future.

There are two main causes of condom breakage.

One is not using enough water-based lube. Not using enough (or any) lube causes friction that may break the condom. Using plenty of lube is particularly important when engaging in anal intercourse, because of the tightness and lack of natural lubrication. If you don't have any water-based lube to hand, use plenty of spit. It's better than nothing and better than using something that may contain oil. Oil is a NO-NO with latex condoms.

The other is having an air bubble in the tip. When putting a condom on, you need to pinch the tip between the finger and thumb of one hand, while rolling it down with the other.

Once it's on, give it a firm stroke from tip to base while watching the tip. If there is air in the tip, you'll see it. If there IS air in the tip, roll it back up and try again.

If it's dark and you can't see the tip well, when you do (what I call) the "stroke test", when you get to the bottom of the stroke, keep a firm grip of the condom and penis at the base and touch the tip with your other hand.

If there IS air present, you'll feel the bubble making the tip stand up and away from the head of the penis. If there IS NO air, the tip will be sucked up tight against the head of the penis.

Once you're sure there is no air bubble in the tip, apply plenty of water-based lube and go for it.

Dear Ann tks once again for the detail explanation, its hard to remain objective with this type of incidents, specially taking these strong meds..! I will keep you posted of the results for your records, however If I may ask the last questions, besides the somehow low risk, plus adding Pep will I reduced even further the chances of coming out well....?
Would it be stupid to stop Pep at 10 days of finishing, I have the feelings I'm really screwing my digestive system plus not functional on my day to day activities, don't feel like doing sports so on........Is the cinnamon tea with honey not recommended during pep? I'm doing Peppermint and much better..!

Cinnamon isn't a problem and neither is honey. Your digestive problems will stop within a day or so of stopping PEP.

It's up to you whether or not you continue with the PEP. I fully expect you to test negative either way.

Thank you Ann, I keep reading over and over your answer in order to remain positive!! Its a tough decision however I will try to finished the treatment 7 more days!!! My hair is fallen badly, my skin is getting weird, I start taking Taural Ranitidine for my stomach, apparently is an anti-ulcer, reversible inhibitor of I don't know what? No idea if it will interact with meds? They told me not, but at this time I have no idea wtf...., I'm concern about how much damage this meds will do for my organism on the long run? This meds are not for me period, but somehow grateful for at least been able to receive it, even if I didn't qualify for it, due to questionable exposure.
I will keep you inform if that's ok with you, will try to finished treatment and forget about everything at least for 3 more month till testing.

Keep it simple. You have a few more days to get through of the meds and then the waiting period to get tested. You will need to make a real effort to focus on other matters in your life.By doing that you may be surprised at how much easier the waiting can be. Really.

The bottles are packed with a standard 30 days worth of dosages, because most people taking the meds don't have the luxury of only taking them for only 28 days. We have to keep taking them.

You can stop at 28 days.

Tks Ann for your kind replies, and very disappointed with the Meds side effects, I hope the new drugs are much better for the people diagnosed, I can't imagine having a normal life taking the meds at least for me I would quit no doubt.
However I'm grateful for having them.
Not sure if I will have to performed a special analysis for my digestive organs? Liver kidney etc, I think this has done severe damage to me..! Is that normal? Any idea of documented damage done to the organism after Pep? The Meds they gave me are from the 90,s I guess, except kaletra.
Last question I will do my follow up test after 3 month post pep, however if i may re ask, what if the girl i did have the incident test negative after 4/5weeks of our incident? Would that some how be a good sign for us?, she has been insisting that she wants to test even for his own piece of mind!
Could you please advise, I know it's impossible to find your status with someone else test!!

The side-effects you are experiencing normally get better after the first two to four weeks (for people who have to keep taking them). For some people, it can take up to six or eight weeks for the body to get used to the meds and stop having such severe side-effects.

If these meds or their side-effects commonly caused long-term damage, we wouldn't advise positive individuals to push through the side-effects, but that's exactly what we do because it's just a period of adjustment.

You have not been given meds that will cause long-term damage in the short period of time for which you are taking them. You really need to calm down and stop over-thinking all of this.

Your side effects should disappear within a day or three of stopping the meds. So again, calm down already. Do you seriously think you would be given meds for this LOW RISK situation that were going to leave you permanently damaged? Think again.

If I've read your previous posts correctly, you only have about two or three more doses of PEP. I fully expect you to be feeling better (side-effect wise) by the middle of next week at the earliest, and by this time next week at the latest.

Yes, her negative result at 4/5 weeks after your incident means that she is unlikely to be hiv positive. Even if she IS hiv positive, you are still highly unlikely to end up positive yourself over this brief, insertive incident, with or without PEP. I have told you this repeatedly and my opinion on the subject is NOT going to change, so stop asking that question.

I fully expect you to ultimately test conclusively hiv negative and I also fully expect your side-effects will stop, with no permanent damage. Seriously.

Make sure you don't find yourself in this position again by using plenty of lube and doing the stroke test. A correctly used and lubed condom rarely breaks.

Hi Ann, hope you're doing well, just to update you we did our last HIV test this Thursday with the lady I did have the accident? It was 32 days after the incident and 4 days after I finished pep!!!
Both test came negative? Is that good news? I wish it was 6 weeks after the exposure.
Any thoughts?

Any negative result is a good result - although in your case it is not conclusive. Sorry, there are no short cuts to hiv testing.
