I'm just curious. My doc told me that it usually doesn't begin to rebound until the VL is really suppressed.

Nonetheless, I got the first of my labs after being on Atripla for 1 month.

My CD4 went from 316 to 345, % went from 27% to 35%. Considering my CD4 had dropped over a hundred to reach 316 I kind of wanted a BIG rebound. I guess I was a little dissapointed until my doc told me the %, which seems like a nice hop. I'll get my VL next week.

Also, he said I didn't have any resistance to what I am taking. I was a little confused though. He said there were some minor NRTI, NNRTI, and PI resistance, and that they wouldn't effect what I was taking or switching to anything in the future. I pressed him for more answers. He said they have a database of thousands of mutations and to not worry, that the results were good. He seemed very confident and definitive about this, so I stopped bugging him about it. It's kind of nagging me though. He answered all my questions but I'm just curious what those mutations are. I figure I can look them up. Perhaps I just need to read more about this stuff.

Congrats on the great results. I would say that you got a nice little bump up in 1 month! The absolute count might not seem big, but the percent is very respectable. As for your resistance results -- if you haven't already, ask for a copy of these results. I get copies of all lab results and keep them, so they are handy if I need them. I also keep a spreadsheet so I can see changes at a glance -- but I am a bit more neurotic than most about this -- probably because I used to work in clinical labs for over 12 years.

You're doing a hell of a lot better than me. It took over a year to get to 207 then it only took another 3 month to drop back down in the 90s. I have an OI that hasn't been controlled keeping my CD4 count down. So yes you are doing well and will be doing a lot better when you VL is undetectable.

Hey Brian,

I'm in a drug study, and am closely followed. Here's what my CD4 and VL did after starting HAART:

CD4 CD4% VL
Start: 262 14% 52670
Day 10: 526 18% 6367
Week 4 607 21% 1291
Week 8 1202 15% 1039
Week 12 1467 25% 149
Week 16 816 25% 288
Week 20 848 24% 139
Week 28 648 25% <50

As you can see, my CD4s rebounded before my VL was completely suppressed. The really high CD4 counts came during an episode of immune reconstitution syndrome when my white counts were extremely high, and so the CD4s were skewed high by the high white counts (because of the way they calculate the absolute CD4 count).

Looks to me like you're doing pretty damn good, with a percent rise like that. I wish my percent was that high, but I'm happy with what I've got. At one point my percent was 11.

I agree with bocker that you should get a copy of your resistance profile to keep. It's more important to you than to anyone else. You should have a copy of it.

I started Harrt in 1996. I went from CD4 335/Vl 225 000 (%N/A) to CD4 625/Vl <50 after 2 months of therapy.
latest results: CD4 785/Vl<50/%41%

I probably took me four years to climp from around 180 to the high 300's, but that was pre-PIs so I guess that was to be expected.

Hello Brian,

I started a from a critical point, back in October of 03...With 16 t-cells and a viral load over 500,000.

As of July my results were cd-4 322 and viral load <50. It also took what seemed to be a very long time to get to that point, but I feel fortunate, that I am at least where I am currently at.

Next blood test are scheduled October 11,06, will see what those blood test results bring. Ity waill also be just about three years on pretty much the same regemin. Other then that, I am doing fair to midland !!

Your doing alright Brian...Your percentages are great,I am currently at 11%,... Got a few percentage points you want to loan me !!

I went back, back in July, from a cd4 of 52 and vl sooo high to a cd4 of 299 3 weeks after starting meds and a vl of 1900. I now a little over a year later have a cd4 of 508 and vl<50(that was July) went Monday for bloodwork and go back on the 2nd of October for results.)
I had my testing done for my resistance and I have no resistance to meds.

Franfrog, those are amazing good results! People usually have an increase of 100 cd4 cells in their forst year on HAART.
Brian your percentage has climbed 8 points in one month. And you are above the 32% which is considered normal. You are doing great!

I started in a Crixivan/AZT trial in 94 with a CD4 of 79 and 4%.
My VL reached undetectable within the first 2 months, but it took just under 3 years to break the 200 CD4 and 13% mark.

I went undetectable very quickly (4 weeks) since my vl was only 10-12k. The CD4 is very slowly moving and has not moved since my last blood work 3 months ago. I'm still at 351 which is up from 289 when I went on meds in April. I was sick with a 24 flu when I had the blood drawn so that might be the cause. My next blood work is in November.

I'm on Sustiva (still) and Truvada.

Thanks for all your responses. I do feel like I must be heading in the right direction. And as with everything HIV we all seem to respond differently.

I still wonder about the resistance test, and I won't be able to get a copy anytime soon (I guess I could call and go pick it up next week). But if there's mutations listed I can't really say I have NO resistance to meds even though my doctor kept saying it's nothing to worry about. Do we often have some random mutations on the front end that are listed but largely meaningless? Or could these be minor mutations that could turn into less minor mutations? I can be worrier at times.

it took 2 years and different meds to get it right at least where i see it as right.in the beginning i was at, cd4, 275, vl 150000 or so.to just the other day, cd4, 599, vl, undetectable,eat right try to stay healthy,exercise if possible,and stay as stress free as possible,and things should help your numbers,have a multi vitamin too.hope your getting along ok,take care

The advances and changes in treatment are really becoming more pronounced these days.

Back when I started meds, we had few choices. We also started treatment when our CD4s dropped below 500, not 350 as is the standard today.

That said, my CD4s have never dropped below 400. I started meds when they dropped to a low of 440. They bounced back quickly, first to the 600 range, if memory serves, then up to a more comfortable 900 to 1,100.

We didn't have percentages back then. Viral loads had only been approved as standard of care about six months before I started meds.

My first viral load test was done in July 1995, when it was still called "experimental" and nobody really understood the ramifications of the test.

As I remember, my first viral load was between 12,000 and 13,000. At that time, I had been positive for 10 years.

When I started meds in March 1996, my viral load had increased to the 60,000 range. I guess my timing was right.

By July 1996, my viral load was undetectable and has, thankfully, remained so since that time.

What about no rebound? My Uncle has been on HAART for 7 years and has never made it above 200 cd4 count. His VL is undetectable. Is there any study to help explain why some people don't have a rebound?

I started meds in the low 200's cd, within two years I was at 900, now at 1440, nine years on meds.

Started med's 18 months ago had a CD of 22 and a VL that was off the charts

Took six months to get to undetectable.
Took three weeks to get to a CD4 of 190
Took six months to get to a CD4 of 300
Took 12 months to get to a CD4 of 650
Took 18 months to go back down to a CD4 of 450.

VL has stayed undetectable. My doc was not overly concerned with the drop, but it worried me, we shall see when I get bloodwork again in NOV.

I suppose the rebound depends upon the individual. I have been HIV + for 19 years and my T-Cells have been between 100-300 for most of that time. My viral load is undetectable - so the regiment is working. However, I have never gotten a big bounce or a steady increase in T-Cells.
