Discontinuation of PCP prophylaxis with CD4 < 200

Patients with undetectable viral load, but a CD4 cell count below 200 may be able to stop PCP prophylaxis:

Antiretroviral-treated patients with sustained virological suppression, but a CD4 cell count below 200 cells/mm3, may be able to safely discontinue prophylaxis for pneumocystiis jiroveci pneumonia (PCP), according to the results of a small Canadian study published in the August 20th edition of AIDS.

Interesting findings. However, it's only a small study (19 patients, 13.7 months off PCP prophylaxis).

My doc had the obligatory, "Well you've been above 200 (barely) for the past six months, so you could discontinue blah, blah blah...." talk with me a couple of months ago.

We both got a good laugh out of that one, because there's no way i'm gonna stop it.

Besides, it wards off the evil MRSA lurking about.

Had the same chat with my doctor, over and over for the last 3 yrs. But since only once did my counts go over 300 (and that was only to 311 before going back down to 264 a month later), I'll stick with my Bactrim. It doesn't make me puke (like many of my other meds do) and it keeps me out the hospital with PCP, so it's one of my favorite meds to take. LOL
